Job overview
The Transfer of Care Hub within Colchester General Hospital is looking for a Lead Case Manager to join their developing team.
This is a key role in supporting the senior case managers and case managers in the implementation and promotion of the discharge to assess pathway across all divisions. Working closely with the discharge and ward teams to standardise process and improve patient flow. Supporting developments within the team to improve the patient journey and reducing length of stay.
This is a fixed term/secondment position until the 31st December 2025
Internal applicants currently employed by ESNEFT will be offered on a secondment basis only.
Main duties of the job
You will delegate and supervise work of senior case managers and case managers to ensure service delivery is standardised to a high level ensuring a responsive and Quality service is maintained.
You will be expected to regularly support surveys, audit, analysis or research to support service development and ensure data quality.
The Lead Case manager will work closely with members of the clinical and non-clinical teams across the Trust in developing policies and procedures to ensure seamless management of patients across all departments
You will participate and support recruitment and selection processes of senior case managers and case managers participating in shortlisting and interview process

Experience
Essential criteria
- • Comprehensive experience at band 4 case manager level or similar post
- • Evidence of continuing professional development
- • Held senior post in a public facing role
- • Experience in supervising or managing staff
Desirable criteria
- • Previous experience of a senior case manager
- • Experience of working in an acute hospital setting
Qualifications
Essential criteria
- • Good general education
- • Leadership courses
Desirable criteria
- • Any relevant vocational qualifications ( NVQ )
- • Evidence of Previous further study
Knowledge
Essential criteria
- • Understanding of the DOH Hospital discharge Policy – including discharge pathways / criteria to reside
- • Knowledge of the Mental Capacity Act and implications for discharge
Desirable criteria
- • Understanding of medical terminology
