- 1 x Permanent Full-Time Post (37.5 hours per week)
- 1 x Fixed-Term Full-Time Post (37.5 hours per week) until 31 March 2027

About the Role
As an Administration Officer within the Care Coordination Centre, you will be at the heart of service delivery, helping to ensure that patients receive the right care, at the right time, and in the right place.
The Care Coordination Centre acts as a key point of contact for patients, carers, relatives, healthcare professionals, and partner organisations. You will provide an efficient and professional administrative service while supporting effective communication and coordination across community healthcare services.
Main duties of the job
The postholder will be responsible for:
- Acting as a central point of contact for calls and referrals into service
- Coordinating care and communicating effectively with clinicians, patients, carers, and partner agencies
- Maintaining accurate and timely records using Trust clinical systems
- Supporting multidisciplinary team working to ensure seamless patient journeys
- Escalating concerns appropriately and contributing to service flow and patient safety
- Providing excellent customer service in a fast‑paced and pressured environment
- Supporting service audits, data collection, and performance monitoring where required
