Job overview
This is an exciting opportunity to join a dynamic and patient-centred heart failure service at Princess Alexandra Hospital NHS Trust. As a Heart Failure Specialist Nurse you will play an important role in improving the diagnosis, management and overall care experience for patients living with heart failure.
You will be working closely with multidisciplinary teams to facilitate smooth inpatient pathways, support early discharge and deliver high-quality nurse-led follow-up in clinic. You will act as a key clinical resource, providing expert advice, education and support to patients, families and healthcare professionals and ensuring care is aligned with national guidance and best practice.
The post also offers the opportunity to contribute to service development and audit, including the National Heart Failure Audit, and to help shape integrated care across hospital and community interfaces. You will also play an important role in supporting advanced care planning for patients with advanced heart failure.
This is an excellent role for an experienced and motivated cardiac nurse who is passionate about delivering high-quality, evidence-based heart failure care and driving service improvement.
Main duties of the job
- To work with health professionals across Princess Alexandra Trust Hospital and all interfaces to improve the awareness and diagnosis of heart failure, and to ensure appropriate systems that underpin good practice are in place.
- To provide care and support to heart failure patients/families/carers in secondary and primary care settings, utilising case management where appropriate.
- To liaise with MDT’s, and Heart Failure Teams in Primary Care to ensure a model of care that will provide a comprehensive care package to all heart failure patients in secondary & primary care.
- Complete audit data specifically the National heart Failure Audit.
- To lead in obtaining, interpreting and action upon patient investigations as appropriate and within agreed clinical protocols recognising when and where to seek advice.
- To ensure provision of high quality nurse led follow up both on the ward and in clinic focussing upon information, patient education around prevention, compliance and concordance with current guidance.
- To ensure all patients when admitted or readmitted to the acute Trust are reviewed to ensure that their medications are optimised to reduce the LOS and repatriate home under the supervision of primary care teams.
- To provide care and support to patients through education to optimise their potential and improve and maintain the best possible quality of life.
- To lead and co-ordinate the provision of palliative care support for patients with end stage heart failure

Experience

- Minimum of 4 years post registration experience some of which must in a cardiology setting and or primary/acute experience
- Experience in cardiac care

- Experience in heart failure care in either primary or secondary care
