Job overview

 

We are looking for a clinical pharmacist to join our pharmacy team on our Adult Assessment Unit (AAU). AAU provides an exciting, varied environment to work in and allows you to develop and utilise your clinical and interpersonal skills. 

You will work as part of the multi-disciplinary team (MDT) alongside senior clinicians, junior doctors, nurses and AHPs, optimising medicines and having a positive impact on the patient experience. Candidates must be well-rounded pharmacists with excellent team-working and communication skills with clinical hospital experience. In addition to providing a comprehensive pharmacy clinical service, there are opportunities to participate in MDT meetings, service improvement projects and training of pharmacy staff and other AHPs.

As a team, we can support your personal development in a supportive environment; this may include completing postgraduate diplomas and independent prescribing courses. We are also open to recruiting to this post as a band 6 to 7 progression where applicants who may not yet have the necessary experience can start this post as a band 6 and after meeting agreed criteria, can subsequently progress to a band 7.

Main duties of the job

Main duties include:

· Ensuring medicines reconciliation is completed for patients admitted onto the assessment unit

· Clinically reviewing new and existing medication and working with doctors to ensure patients are on the most appropriate medication as per local and national guidelines, including deprescribing where appropriate

· Communicating with primary care colleagues to ensure smooth transition of care

· Ensuring prescribing is as per Trust formulary

· Working as part of the MDT

· Supporting the training and development of pharmacy staff, junior doctors and other AHPs

· Providing clinical support to the Same Day Emergency Care (SDEC) unit

· Undertaking audit and project work as part of service review/service developmen

  • Relevant post-registration experience
  • Experience working on a busy hospital ward and working as part of an MDT
Desirable criteria
  • Experience of participating in audit activities

Qualifications

  • Vocational masters degree in pharmacy
  • Registered with General Pharmaceutical Council (GPhC)
  • Postgraduate clinical qualification (over and above that of the 1st degree, preferably at Masters level) and/or significant relevant clinical experience to demonstrate a similar level of knowledge
