An exciting opportunity has arisen for an experienced, motivated and proactive Paramedic to join our Reactive Emergency Assessment Community Team (REACT).  

REACT is an innovative multidisciplinary community service within ESNEFT, bringing together Urgent Community Response (UCR), Hospital at Home, OPAT and Out of Hours pathways to deliver high-quality, patient-centred care closer to home. The service works closely with the East of England Ambulance Service, Hospital at Home, Integrated Neighbourhood Teams and acute services to support admission avoidance and deliver rapid intervention for patients experiencing acute deterioration in the community.

You'll play a key role in delivering the national 2-hour Urgent Community Response standard, supporting patients to remain safely in their own homes wherever appropriate and promoting our Home First philosophy.

This role offers the opportunity to influence the development of innovative community pathways, strengthen system-wide relationships and contribute to the ongoing transformation of urgent community care services across East Suffolk.

The role is available for up to 37.5 hours per week and includes participation in a 7-day service operating between 08:00 and 20:00, including weekends and bank holidays.

Main duties of the job

· Provide expert paramedic assessment, clinical decision-making and intervention for patients with acute illness, frailty and complex health needs within their own homes.

· Support admission avoidance through rapid assessment and timely intervention, ensuring patients receive the right care, in the right place, at the right time.

· Triage and prioritise referrals, including those received through the East of England Ambulance Service, ensuring delivery of the national 2-hour Urgent Community Response standard.

· Provide visible clinical and operational leadership during shifts, supporting patient flow, safe caseload management and effective deployment of resources.

· Act as a clinical role model and professional leader, supporting supervision, appraisal, education and development of staff across the multidisciplinary team.

This is an exciting opportunity for a Paramedic looking to work at an advanced level, combining expert clinical practice with leadership, service development and multidisciplinary collaboration. You will work autonomously whilst providing clinical leadership across the service, supporting staff development, patient flow and operational delivery.
