Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.
Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust. There are also three main community and mental health service providers who work together through a community collaborative.

Job overview
We are seeking a highly motivated, enthusiastic and experienced Band 4 Therapy Assistant to work in the Discharge to Assess service. You will be a part of the therapy Service and will work alongside Specialist Occupational Therapist and Physiotherapists to deliver a assessment at home.
The aim of the service is to create a smooth pathway from the acute hospital into the community by assessing the patient's therapy and care needs in their own home. This means that patients no longer need to wait in hospital for these specific assessments which will reduce delays discharges, improving patient flow, and allow patients to go home earlier.
Southend hospital and we support discharge from hospital and compete an assessment at the patients home. Patients are still assessed in the hospital, however discharge to assess is an extension of this to ensure patient has the right care and equipment at home.
The team fosters a fun and supportive environment with focus on developing band 4’s to feel confident in there assessment skills. This community based approach require lone working alongside a team based approach to assessment.
***Due to the nature of the role the successful candidate would be required to be a car driver with access to a car for work purposes and lone working as part of the role. The job require a dual work based environment – working in patient home and completing admin based task from home based environment (not office based) ***
Main duties of the job
The Band 4 Therapy Assistant will take responsibility for the management of a defined caseload of patients within the D2A pathway following triage by the qualified therapist.
- To work as part of the team providing a comprehensive Therapy service under the guidance of qualified staff for patients in their own homes.
- Equipment provision to support people to be more independent and safe at home.
- Support people to reable and recover at home, as capacity allows.
