Lung Cancer Patient Navigator

Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.

Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust. There are also three main community and mental health service providers who work together through a community collaborative.

We collaborate with other valuable partners, including three Healthwatch organisations, nine voluntary and community sector organisations and three top tier local authorities, with seven district, borough and city councils, as well as our local universities.

 

Job overview

An exciting opportunity has arisen for an enthusiastic and dynamic individual to join our team as a Lung Cancer Patient Navigator.

You will support the Respiratory service to facilitate the smooth running of the patient journey, by ensuring that the lung cancer pathway is coordinated, streamlined and all processes revolve around the patient and their individual needs.

The role of the lung cancer patient navigator is to provide specialist knowledge for the pathway of lung cancer patients . This is a 12 month Fixed post due to funding.

 

Main duties of the job

If successful you will work directly and indirectly with patients and their carers, and be responsible for capturing information about patients, ensuring up-to-date relevant information is recorded on the patient record and Trust systems.

You will have a good knowledge and understanding of patient complete pathways. As a key requirement of this role, the post holder will have excellent communication and organisational skills, together with a high attention to detail and levels of accuracy.

Qualifications

Essential criteria
  • NVQ Level 4 or equivalent experience
Desirable criteria
  • Relevant qualifications in healthcare administration, cancer services or leadership

Knowledge & Experience

Essential criteria
  • Proficiency in IT systems, including fast and accurate keyboard skills
  • Previous administrative experience within a healthcare setting
  • Experience of applying hospital administration systems
  • Knowledge and understanding of key access targets i.e. 62 day cancer and 28 day FDS
Desirable criteria
  • Experience working within cancer pathways
  • Previous experience working with data input
  • Understanding purpose of PTL's
