Job overview

We have an opportunity for a full-time Care Co-ordinator to join our  Fair Havens Community Team, located at our Fair Havens Hospice, adjacent to beautiful Priory Park in Southend on Sea.

As this role supports a  7-day service, there is a requirement that candidates be available to work on a rostered basis, across the 7 days, including weekends and bank holidays.

Main duties of the job

You will work as part of the wider team of coordinators within our Fair Havens Community Team, to ensure that our processes and HRAS referrals are streamlined with robust internal pathways, as well as undertaking administrative duties as required.

Detailed job description and main responsibilities

  1. To be responsible for data collection for all aspects of placements utilising SystmOne records and computerised databases 
  2. To work closely with clinical staff within the Core teams; to deliver the above service and to escalate clinical concerns and sourcing difficulties to the Rapid Access, and to the Head of Adult Services/Community Lead.  
  3. To support the Rapid Assessment and Discharge Team with administrative support as required 
  4. Through telephone, virtual and face-to-face discussions, provide information to patients with palliative and end of life care needs, supporting their families and carers, liaising with clinical professionals and referring on as necessary to appropriate health and social care partners 
  5. To support the Hospice in building strong relationships with external partners and stakeholders and promote the good reputation of the Hospice e.g. local trusts, ICB, health and social care partners, businesses and voluntary organisations 
  6. Torole model thevalues of Havens Hospices and support others within the organisation to do so byrecognising positive behaviours,challengingpoorbehaviour, encouraging and supportingchange 
  7. To be visible and credible with the ability to listen and respondto staffand topatients’ and family issues and insights 
  8. To communicate effectively with all staff across the organisation to promote one team, one vision and collaborate with colleagues as appropriate to deliver this 
  9. To work as part of the Rapid Access to Care Team and provide cover during the absence of other team members 
  10. To support the Rapid Access to Care Team in producing personal care reports and activity data 
  11. To demonstrate negotiation skills when dealing with difficult or challenging situations such as managing patient/carers unmet expectations, reporting conflicts to your manager 
  12. Tomanage your own performance as well as identifying existing skills and competencies and ensuring that personal development plans are tailored to the changing capability requirements of the organisation 
  13. To support the Clinical Quality Team in robust collection and collation of performancedata to ensure continuous improvement of services 
  14. To collaborate with Fair Havens Community Lead Nurse in streamlining processes and procedures within the team using quality improvement methodology and meaningful clinical outcome measures 
  15. To ensure contracts are prepared and signed for following standard operating procedures 
  16. To negotiate best value for money regarding placements 
  17. To escalate to clinical teams or your line manager any clinical or operational issues in a timely fashion 
  18. To input the required administrative information 
  19. To ensure personal care data entry and collection and support presentation of such data to meet quality requirements 
  20. To train other relevant staff e.g. Navigators and Administrators, in Nursing Home procurement and data entry requirements 
  21. To work with the Service Lead to establish, implement and participate in systems and processes that support the service 
  22. To use communication skills in delivering information clearly, sensitively and unambiguously with staff, patients, families and professionals by face-to-face, telephone and/ or electronic means

Essential criteria
  • 2 x A' Levels or equivalent or demonstrable experience in a similar role

Skills & Experience

Essential criteria
  • Demonstrable experience of administrative or clerical procedures and systems.
  • Excellent Microsoft Office and IT skills, including Excel, PowerPoint, Word, Outlook
  • Demonstrable office management and administration experience.
  • Excellent written and verbal communication skills
  • Experience of updating and maintaining records, databases, and other systems accurately and efficiently.
  • Experience of working within a clinical team
