Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.

Job overview
An exciting opportunity has arisen to join the Frailty Team at Mid and South Essex NHS Foundation Trust, Southend Site.
The successful applicant will join the existing Frailty Team to provide expert advice, assessment and plans of care for frail patients within various departments and ward areas.
We are looking for an experienced Nurse/Allied Health Professional who is highly motivated and who has a commitment to the delivery of high quality and safe care to our frail patients. You will be organised and able to prioritise your own workload. You will be able to communicate effectively with staff at all levels of the organisation and have excellent leadership skills.

Main duties of the job
Perform comprehensive assessment of the needs of frail elderly patients.
You will utilise a range of assessment skills including history taking, interpreting laboratory results, imaging and physical examination, within the boundaries of the role and personal competencies. Plan, implement and evaluate care delivery according to the changing health needs. Liaise with the DME Senior & Consultants colleagues / Other speciality teams, where necessary to ensure safe and appropriate plans in place.
Management plans may include non-medical prescribing.
Attend Emergency Department/assessment areas to identify & assess patients suitable for transfer to the Frailty unit or Elderly Short Stay Unit (ESSU).
Demonstrate Advanced Clinical Skills in assessment &/or diagnosis &/or treatment including non-medical prescribing.
Involve patients and carers/relatives in the planning and delivery of care and development of services.
Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to on-going care needs and discharge planning.
Establish and maintain effective communication with patients and carers/relatives, and professionals across health and social services.
Represent the Frailty Team at weekly MDTs both in person and in virtual meetings.
This role involves regular / daily direct patient care, of whom may have acute and complex health needs.
