Breast cancer Pathway navigator

Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.

Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust. There are also three main community and mental health service providers who work together through a community collaborative.

We collaborate with other valuable partners, including three Healthwatch organisations, nine voluntary and community sector organisations and three top tier local authorities, with seven district, borough and city councils, as well as our local universities.

 

Job overview

The role of the cancer patient navigator is to provide specialist knowledge for the pathway of cancer patients across Mid and South Essex NHS Foundation Trust.  The role will facilitate the smooth running of the patient journey by ensuring that the pathway is co-ordinated, streamlined and all processes revolve around the patient and their individual needs. 

You will work directly and indirectly with patients and their carers, and be responsible for capturing information about patients, ensuring up-to-date relevant information is recorded on the patient record and Trust systems.

You will have a good knowledge and understanding of patient complete pathways.  As a key requirement of this role, you will have  excellent communication and organisational skills, together with a high attention to detail and levels of accuracy.

You will support and work within a multidisciplinary team consisting of nursing, medical and operational colleagues. Have an ability to engage and communicate with diverse audiences will be needed to support in the transformation of services to improve patient outcomes and waiting times.  

The role will have a primary base at one of the three hospitals but will coordinate patient pathways across the MSE network.

Main duties of the job

To be a key resource, having up to date knowledge, skills and training to validate and co-ordinate the breast cancer patient pathway.

Work autonomously and manage workload independently, seeking advice and support when necessary from key personnel, adhering to current guidelines for information provision and confidentiality.

Be a main point of contact on the breast cancer PTL.

Work closely with Breast operational services across MSE to ensure timely navigation of patients through the pathway.

Deliver key agreed actions within specified timescales.

Demonstrate effective communication with clinicians, nurses, managers, patients, carers, other health professionals and stakeholders.

Attend MDT meetings and pick up any actions that arise as part of the MDT outcome.
