Job overview

Are you a real collaborator & able to work alongside partners & lead the transformation of a multi-agency pathway for children & young people?

Are you a registered clinician with leadership & change management skills, alongside experience of working in children’s services?

An exciting opportunity has arisen to provide leadership to an ambitious multi-agency programme to transform how children’s services including Education Inclusion, Early Help, Voluntary & Community sector & mental health services work together. The current pilot has enabled more joined up working at the point of access & beyond to better support mental health, wellbeing, education & other needs of children, young people & their families in Gloucestershire.

We are looking for an enthusiastic Lead to continue to develop a Multi-Agency Single Point of Access, working with key stakeholders from across the mental health, education & the wellbeing system in Gloucestershire to shape a long-term, sustainable joined-up referral front door for children & young people’s mental health and education service provision.

This is a real chance to drive change to improve outcomes for children & young people in Gloucestershire. The right person for this role will bring strong experience of change management, leadership & the ability to work at pace to deliver results; a strategic approach; a high level of communication, negotiation & influencing skills. Project management support will be provided to the successful candidate

Main duties of the job

  • Continue to develop how Gloucestershire incrementally moves towards joined-up working and decision-making for referrals at a Multi-Agency Front Door.
  • Be familiar with current working processes around the county, specifically around referral management for mental health and emotional wellbeing, Education Inclusion and Early Help services.
  • Work with Education, Health, Early Help and VCS colleagues to ensure a high quality of decision making at point of referral, ensuring the right services are genuinely collaborating together.
  • Work alongside the Parent/Carer Forums, GCC Ambassadors and CAMHS Experts by Experience to ensure delivery is centred around the voice of the child and young person.
  • Manage the implementation of agreed processes for a county-wide model with the outcome of reducing children being “ping-ponged” around services.
  • Improve service experience for families and ensure that children and young people experience a personalised approach for multiple and complex needs where required.

Working for our organisation

At Gloucestershire Health and Care NHS Foundation Trust, we are supportive of the people we care for in the community and aim to provide a fully responsive service to everyone who needs it. This is the same approach we take for our employees. We look to value the skills and experiences of those we work with, whether they are service users, carers, families or those looking to work with us.

Gloucestershire Health and Care NHS Foundation Trust is part of the NHS. We treat NHS patients according to NHS principles and standards. The difference is the Trust is accountable to local people, rather than to Government, and so we are able to work closely with our community to develop services in the way that best suits the needs of local people.

Our Staff Survey results showed people at our Trust feel they are engaged, part of a team and work for a compassionate and inclusive organisation.

Our overall scores were better than average in eight of nine themes and level in one - the best overall results for a community, mental health and learning disabilities Trust in the south west and first equal amongst all sector NHS providers within the region too.

Detailed job description and main responsibilities

Gloucestershire Health and Care is committed to embedding and providing personalised care through all of its services as part of our high-quality care strategy. The purpose of personalised care is to empower people to lead the lives they want to live. Personalised care starts with a conversation with people about what matters to them. It builds on what people can do, as well as addressing their health needs.  Including new ways of working and delivering care, emphasising that we need to make effective use of the full range of our people’s skills and experience to deliver the best possible patient care.  So we want to know What Matters to You and that you will share our values and join us on this very important journey.
