Job overview

The Gloucestershire Respiratory Service delivers a comprehensive range of treatments to support patients living with Chronic Obstructive Pulmonary Disease (COPD), Asthma, Bronchiectasis, and Pulmonary Fibrosis.

Our countywide, integrated service works in collaboration with both primary and secondary care. We offer:

  • An accredited Pulmonary Rehabilitation Programme
  • Home Oxygen Assessment and Review Service
  • Specialist Physiotherapy Clinics
  • Complex Respiratory Assessment and Review Clinics

We are seeking a self-motivated and innovative Nurse or Physiotherapist to join our dynamic multi-disciplinary team. This is a varied and rewarding role that offers the opportunity to make a real difference in the lives of patients while actively contributing to quality improvement and service development across Gloucestershire.

Main duties of the job

This is a highly varied and patient-focused role, ideal for someone who enjoys working both independently and collaboratively. You will be expected to travel to different locations across the county, including patients’ homes, clinics, and virtual settings.

Key responsibilities include:
•    Delivering respiratory care across a range of settings including home visits, community clinics, and virtual sessions

•    Supporting the delivery of Pulmonary Rehabilitation programmes, including education and exercise classes

•    Performing assessments and reviews for patients requiring home oxygen therapy, including the use of blood gases to guide treatment

•    Working closely with acute hospital respiratory teams to provide integrated care

•    For Physiotherapists: managing patients with complex presentations such as breathing pattern disorders and sputum retention

You will also have the opportunity to rotate across the four arms of the service depending on your professional background and clinical experience:

1.    Home Oxygen Assessment and Management
2.    Respiratory Assessment and Treatment Clinics
3.    Pulmonary Rehabilitation
4.    Specialist Physiotherapy Clinics

The respiratory service specialises in caring for patients with Chronic Obstructive Pulmonary Disease (COPD), Asthma, Bronchiectasis and Pulmonary Fibrosis.

Detailed job description and main responsibilities

Clinical 

  • Provide assessment and development of individual care planning aimed at improving the outcomes for respiratory ill patients either in their homes or in clinics 
  • Where appropriate review patient medication and make or advice changes as appropriate.  
  • Working within professional guidelines and scope of practice to provide holistic and individualised support to patients and relatives to improve self-management of their chronic respiratory condition. 
  • To travel around the county to assist in delivering Pulmonary Rehabilitation classes with other members of the Respiratory Team. Including giving educational talks to patients who attend the classes. To ensure that all classes are run safely and efficiently and all health and safety policies are adhered to at all times. 
  • Assess and review patients who may require oxygen therapy, which will include taking and analysing Capillary or arterial blood gases, Carbon monoxide testing on suspected smoker.
  • For physiotherapist: Provide specialised respiratory physiotherapies including advanced airway clearance and breathing pattern disorder.
  • Support and manage care for patients recently discharged from either the acute hospital or the Rapid Response service.  
    Work across the clinical pathways offered by the Respiratory service in Gloucestershire to offer an integrated care pathway  
     
    Professional 
    •    Working within professional guidelines and scope of practice to provide individualised support to patients and relatives to improve self-management of their chronic respiratory condition 
    •    Maintain accurate written records to ensure that service user details and details of the care given is recorded in notes line with Trust policies 
    •    To demonstrate excellent Communication skills when feeding back both verbally and in written form in order to support and work alongside other Health care providers such as Primary Health, Social Care, and acute and community hospital teams  
    •    To contribute and at times lead on quality improvement ideas working towards the development of the service.  Continuously looking at ways to move towards a more integrated service across community and acute services. 
    •    Participate in the promotion of the service, attending meetings and promotional events where deemed relevant to role 
    •    Work with and support other members of the Multi-disciplinary team as required 
    •    Attend and contribute professional perspective, clinical supervision at weekly and monthly team meetings.   
     
    Leadership / Management 
    •    Plan and deliver teaching sessions to members of the multidisciplinary team and outside agencies including professions from primary care in order to pass on specialist knowledge and skills 
    •    To facilitate the learning and support of unqualified members of staff and students 
    •    Contribute to service development and quality initiatives in line with local and national policies using audit and policy development 
    •    Undertaking other duties appropriate to the post as requested by senior managers 
    •    To line manage other staff appropriate to role

  • Post graduate experience working with people with respiratory conditions
  • Evidence of teaching and supervision of other staff
  • Hold full UK driving licence and access to own car
  • Experience of working with people with long term conditions

  • Evidence of NHS experience
  • Evidence of working within Pulmonary Rehab

Knowledge

Essential criteria
  • Evidence of relevant professional registration - HCPC Physiotherapy NMC nurse
