Job overview
Gloucestershire’s Integrated care System is on a journey to provide more place based and integrated care for our population, where clinically appropriate. The integration of diabetes services across acute, community and primary care is just one example of this.
The Diabetes and Obesity Clinical Programme meet regularly to oversee the quality and performance of our services, and is committed to delivering high value, integrated care.
We aim to;
· Improve the identification and management of Diabetes and improve management of obesity.
· Reduce health inequalities and improve the health and wellbeing of the population.
· Reduced gap in amenable deaths between the most and least deprived by 2029.
· Reduce the demand on acute care by improved management in the community through delivering integrated care.
This is an established position within our system, integrated with the acute trust team. The post's primary emphasis is on community-based care, offering the successful candidate a key opportunity to further evolve the outreach model for diabetes services.
Main duties of the job
Job Summary
- Lead the Diabetes Integrated Service
- Providing training, education, and support to professionals across the system
- Contribute to system wide pathway review and development
- Conduct community-based outpatient clinics for adults with complex Type 2 diabetes, young type 2 clinics and selected patients with Type 1 diabetes.
- Provide structured clinical governance to the Community Diabetes Specialist Team
- Collaborate with the multidisciplinary team (MDT) including Diabetes Specialist Nurses, Dietitians, a Nutritionist, Psychologist, Health Educators, Primary care Champion, and Podiatrists.
- Work with the Acute Diabetes and Endocrinology team at Gloucestershire Hospitals NHS Foundation Trust.
- Engage with Integrated Locality Partnerships and Primary Care Networks to develop the service.
- Continue and develop MDT and mentored diabetes clinics focusing on education within Primary Care Networks.
- Involvement in various specialised clinics and education programs for diabetes.

Detailed job description and main responsibilities
1. Clinical Responsibilities/Key Responsibilities
The appointee will work in collaboration with the members of the Integrated Diabetes Service and the current Diabetes and Endocrinology physicians to provide continuity and high standards of care. The main focus on the role is to work closely with community-based specialists’ team to manage increased complexity within the community.
2. Professional Development, Education and Training
· The duties of this post will also include supporting primary care to manage the large majority of patients with diabetes not requiring secondary diabetes care input. This will involve liaising with GP’s and Practice Nurses, taking part in education programmes for practice staff, responding to email requests from GP’s and Practice Nurses for clinical advice.
· The appointee will take on the supervision of the GP trainee
· Integrated Diabetes Team: Education, supervision and regular appraisal
3. Management and Leadership
· To provide medical information for the development of systems appropriate for Trust needs
· To participate in departmental consultant and senior staff meetings whilst remaining within the framework of the agreed strategy of the Trust and the Department
· To attend other departmental, Divisional and Trust meetings as necessary
· To attend regional and national meetings as necessary
· To undertake all work in accordance with Trust procedures and operating policies
· To become an active member of the clinical programme group
4. Quality Improvement
· To promote evidence based practice. To respond appropriately to NICE guidance. To change own and others clinical practice in response to new clinical developments
· To undertake and supervise relevant clinical audit topics
· To be involved in risk management and quality assurance
· To remain up to date and engaged in CME/CPD as per RCP guidelines
· To respond appropriately to complaints and critical incidents
· The post-holder will be expected to take part in developing clinical audit in accordance with agreed Directorate and Corporate Clinical Governance programmes.
· Teaching primary care colleagues, GP trainees, junior doctors and medical students, nurses and other paramedical staff in the team in speciality and special interest
5. Accountability
· The post-holder will be immediately accountable to the specialty Clinical Lead / Specialty Director and the Diabetes Clinical Programme Group. Further line management is provided by the Chief of Service for medicine and subsequently the Medical Director
· Co-operate with the commissioning lead on budget issues. The intention is to have a programme budget for diabetes across primary, community and secondary care
The purpose of this post is to provide a substantive Consultant for the Integrated Diabetes Service. As a senior employee of the Trust the post-holder will work in close co-operation with, and support other clinical, medical professional and managerial colleagues in providing high quality healthcare to patients with diabetes registered in Gloucestershire.
The post-holder has a general duty of care for their own health, safety and well being and that of work colleagues, visitors and patients within the hospital. This statutory duty is in addition to any specific risk management or clinical governance accountabilities associated with the post.
Finally, the post-holder is expected to:
· Observe the rules, policies, procedures and standards of Gloucestershire Hospitals NHS Foundation Trust together with all relevant statutory and professional obligations
· Observe and maintain strict confidentiality of personal information relating to patients and staff
· Be responsible, with management support, for their own personal development and to actively contribute to the development of colleagues
6. Clinical Governance
· To undertake all work in accordance with Gloucestershire Hospitals NHS Foundation Trust procedures and operating policies
· To attend accredited conferences and meetings to update personal level of clinical practice, teaching and management skills in line with CME/CPD requirements
· To participate in an annual Job Planning Review process
· To participate in the Trust’s annual Appraisal process and to respond to any identified development needs
· To adhere to the Trust’s Adverse Clinical Incident Policy
· Communication: This will include responsibility for and arrangement of communication and liaison with primary care colleagues, other community services and other medical and service departments within the hospital including: General Management, Medical Records
· Communication with primary care colleagues will include video consultation, telephone discussion of problems and writing of letters about cases seen in the community based clinics
· Communication with other hospitals and special departments in other hospitals

Qualifications and Training
Essential criteria
- Full General Medical Council registration
- On Specialist Register for General (Internal) Medicine and Generic Medicine via CCT or CESR-CP (Combined Programme) (Proposed CCT/CESR-CP date must be within 6 months of interview
- Ability to appraise research critically and apply research outcomes to clinical and surgical problems
Desirable criteria
- Willingness to undertake additional professional responsibilities at local, regional and national levels
- Additional degrees
- Experience in community based care provision
- Diplomas or postgraduate thesis relevant to the specialty
Knowledge and Skills
Essential criteria
- Ability to appraise research critically and apply research outcomes to clinical and surgical problems
- Ability to supervise junior staff undertaking research projects
- Evidence of participation in Audit and Clinical Governance Programmes
- Ability to manage and lead a Diabetes & Endocrine/General Medicine firm

Clinical Experience
Essential criteria
- Clinical training & experience equivalent to that required to obtain UK CCT in Diabetes, Endocrinology & General Medicine

Desirable criteria
- An interest in obesity (weight management)
- Experience in community based care provision
