This is a general nephrology post with a significant GIM inpatient component and a 1 in 6 on call for the renal wards and renal referrals.

As a senior employee of our Trust you will work in close co-operation with, and support not just other clinical professionals but also our managerial colleagues in providing high quality healthcare to our patients.  

Our acute hospital base is Gloucestershire Royal Hospital in Gloucester where our two wards are located including an inpatient acute dialysis service. Two of our outpatient dialysis units are also on site with a satellite unit in Cinderford in the Forest of Dean. There is a 7-day renal consultant presence on the Gloucester site. 

We have good working relationships with our local Integrated Care Board and the local Integrated Care System (One Gloucestershire). A Renal Clinical Programme Group has been established to work together with our local partners to take forward prevention and care of kidney patients of all stages in line with the Renal Services Transformation Programme, GIRFT and our regional Operational Delivery Network in the South West. We are fortunate that we relate predominantly to a single ICB, County Council and Gloucestershire Health and Care Trust who provide community services including mental health and our community hospitals.

The post-holder is expected to develop and lead a high-quality renal service, delivering timely care to patients with kidney disease and medical inpatients. Key responsibilities include:

  • Participating in a 1-in-6 inpatient on-call rota on Ward 7B and Ward 7A (Monday–Friday).

  • Providing input for renal patients referred by other specialties when on-call or attending clinics at Cheltenham General Hospital (CGH).

  • Offering advice on renal cases referred by GPs, pharmacists, Nurse Practitioners, etc., including via Cinapsis and Advice & Guidance services.

  • Running approximately three outpatient clinics weekly across CGH, Gloucestershire Royal Hospital (GRH), and Dursley.

  • Reviewing new admissions to renal wards during on-call weekends, including discharges or patients of concern.

  • Being available to assess renal patients in ED or Same Day Emergency Care (SDEC) during on-call periods.

  • Leading development of Home Therapies, particularly expanding the Home Haemodialysis programme, in collaboration with specialist nurses and the dialysis provider.

  • Contributing to consultant-led procedures, such as medical peritoneal dialysis (PD) tube insertions (training available if required).

  • Ensuring clinical practice aligns with contract terms and the objectives of the Division and Trust

In addition to the main duties above you will be expected to participate in postgraduate training and other staff teaching as appropriate, and to act as  clinical and educational supervisor to junior and middle grade medical staff, subject to relevant training and ongoing educational CPD as agreed in the annual appraisal.

You will have a general duty of care for your own health, safety and wellbeing and that of work colleagues, visitors and patients within our hospitals. This statutory duty is in addition to any specific risk management or clinical governance accountabilities associated with the post.

You would also be expected to:

·         Observe the rules, policies, procedures and standards of Gloucestershire Hospitals NHS Foundation Trust together with all relevant statutory and professional obligations

·         Observe and maintain strict confidentiality of personal information relating to patients and staff

·         Be responsible, with management support, for your own personal development and to actively contribute to the development of colleagues.

·         Undertake an annual appraisal and 5-year revalidation. GHNHSFT also provides a mentor when we take up our consultant post.

·         You will be responsible for the provision of medical information for the development of systems appropriate for our organisational needs

·         You will participate in departmental consultant and senior staff meetings

·         You will attend other departmental, divisional and Trust meetings as necessary

·         You will attend regional and national meetings as necessary

·         You will undertake all work in accordance with our organisational procedures and operating policies

·         You will be involved in the development of our renal services

 

Research and Development

You will be encouraged to be active in clinical research. The Trust is working towards achieving University Hospital Status requiring commitment to high quality training and research. The renal department is actively involved in NIHR approved research including current trials such as EASI-Kidney, PHOSPHATE, RESOLVE and a number of smaller commercial trials.

Clinical Governance

The Trust has a well-established Safety and Quality Improvement Academy which oversees QI work and is able to offer training and coaching in QI techniques.

In the Renal Department we have monthly directorate, audit and Morbidity & Mortality meetings. Our Trust has a well-established framework for clinical governance that incorporates our clinical audit programme.  We are all expected to take part in developing clinical audit activities within the department to achieve clinical governance objectives in our agreed Divisional and Trust clinical governance programs.
