Main duties of the job
Due to University Hospital being established as a regional centre for Alpha-1 Antitrypsin Deficiency (AATD) , an exciting opportunity has arisen within service that forms part of the multidisciplinary team for a Clinical Nurse Specialists with a split role within The COPD Team. This role will allow further contribution to the future progress of health care within the service.
The team is committed to delivering a high quality efficient service for the diagnosis and treatment for patients from the local and regional areas. The service currently operates a seven day per week service and the team work to a rota to ensure the service needs are met. You will be working alongside a team of clinical nurse specialists and clinicians. The team are pivotal in the delivery of respiratory targets. This role is an integral part of the team which co-ordinates pathways across multi-disciplinary teams for patients

Detailed job description and main responsibilities
Duties
Be a point of contact for patients/carers for defined episodes/pathways of care and provide clinical care, specialist nursing advice and psychosocial support.
Accept referrals to agreed criteria, and work in partnership with other professionals to deliver co-ordinated programmes of care effectively and referring patients to other practitioners as required.
Deliver expert nursing care to patients in agreed practice settings, working as part of nursing/MDT teams.
Develop and sustain appropriate multidisciplinary working relationships to improve health outcomes and healthcare delivery systems.
You will provide specialist advice, information, education and support to patients, their carer’s and other health care professionals. You will be managing a patient caseload and will act as a key worker and advocate for patients from the point of diagnosis. An integral part of the role will be to communicate to patients in a way that empowers them to make informed choices about their health and treatment. The post holder will work to agreed professional, departmental and Trust boundaries as required to provide patient-centred seamless care.

Qualifications, knowledge and experience
Essential criteria
- Registered nurse on relevant part of the NMC register Change Management, clinical practice development
- Degree qualification, 120 credits at L6 or possess an equivalent knowledge to be assessed through evidence of relevant specialist training, experience or portfolio
- Significant post registration experience / qualification / training, including experience and knowledge within relevant speciality
- Completion of all Band 5 (preceptorship) competencies
- Higher Education Institute approved course in supporting learners in practice or evidence assessed to be of equivalent experience, demonstrating support to a range of learners in practice. Provide evidence of both supporting a range of learners in practice and supporting colleagues to promote effective learning in practice
- Extensive post registration experience within specialist area
- Knowledge of current issues within the acute health service
- Understanding of personal accountability
