Senior Pharmacist - Emergency Floor

JOB SUMMARY
This senior pharmacist role will lead and support the Pharmacy Service provided to the Emergency Floor (Acute Medical Unit, SDEC, and Emergency Department).
This includes responsibility for the Pharmacy Hub located on the Acute Medical Unit and staff associated with the pharmacy service to the Emergency Floor.
The post holder will be the responsible for the pharmaceutical care to the Acute & Emergency Directorate including clinical, medicines safety and financial governance associate with medicines use.
There will also be operational commitment to the Pharmacy service weekends and Bank Holidays on a formal rota (currently 1:6).

Clinical Responsibilities:
- Will be the Pharmacy lead for medicines optimisation to the Emergency Floor (Acute Medical Unit, SDEC and Emergency Department)
- Responsible for developing and delivering specialist clinical pharmacy and medicines optimisation to the Emergency Floor
- Use specialist knowledge of medicines to advise medical, nursing, patients, carers and pharmacy staff on all pharmaceutical aspects of care
- To receive and deliver highly complex clinical information including information that may be contentious and highly sensitive relating to patient care
- Responsible for the development, monitoring and review of medicines related guidelines to the Emergency Floor
- Provide direct patient facing care as part of the ward based and dispensary clinical pharmacy service through clinical checking of prescriptions in line with professional, legal and best practice standards
- Lead on medicines reconciliation for the Emergency Floor to ensure patients receive the correct medication as early as possible in their stay
- Liaise with primary care and social care colleagues to provide to ensure transfer of information relating to patients medicines is timely and accurate
- Agree priorities for high risk patient groups with regards to their medicines.
- Prescribes appropriate medication within professional competency (including transcribing regular medication and initiation of new medications)

Education & Qualifications
Meini prawf hanfodol
- MPharm & registration with the General Pharmaceutical Council as Pharmacist
- Post graduate diploma in clinical pharmacy or equivalent experience to a similar level
- Post Graduate qualifications, or equivalent experience, relating to Acute/Emergency medicine.
- Non-medical prescribing qualification (or working towards/ willingness to complete)

- Membership of the Royal Pharmaceutical Society of Great Britain
Skills, Knowledge & Abilities

- Able to demonstrate a clear understanding of clinical pharmacy practice relating to Acute/Emergency medicine
- Able to communicate effectively using a variety of methods (written, verbal, presentation)
- Demonstrate a clear understanding of best practice and legal requirements relating to medicines storage, prescribing and administration
- Knowledge, and understanding relating to the preparation of medicines (especially injectable medicines)
Experience

- Significant post registration experience in a hospital pharmacy setting with a proportion of time in a clinical role
- Experience of investigating complaints and medication incidents within an organisation
- Experience of audit within an organisation

- Experience of leading on personnel selection, managing teams and individuals.
