Join the School Nursing team in Hereford delivering the 5–19 Healthy Child Programme as a School Nurse, promoting health and preventing ill health for young people, families and the wider community. 
 

The School Nurse will work within a defined area, assessing need, delegating and supervising work within the team.  They promote early intervention, health promotion, advocacy, social inclusion and the reduction of health inequalities. 

The School Nurse works in partnership with young people, families, schools and communities, primary care and other professionals to facilitate health enhancing activities.  

Main duties of the job

Use evidence based practice to assess, plan, implement and evaluate care in line with the Healthy Child Programme. 

Address inequalities in health and maintain links with relevant agencies. 

Establish networks within the community in order to inform and influence policies which affect health outcomes.
