Job overview

PPT provide evidence based long-term intervention that supports the development and maintenance of capable environments that reduce the risk of placement breakdown/admission to hospital and enhance the quality of life of service users with complex and challenging needs.  Promoting a Positive Behavioural Support approach PPT provide competency-based training and skills development in; Understanding and Responding to Autism (SPELL Framework), Person Centred Active Support and Understanding and Responding to Behaviours that Challenge.  Established in 2019 the team works with service users, their families and community-based support networks including social workers and CLDHT colleagues across Buckinghamshire. Taking internal referrals from clinicians within the Bucks Community Learning Disability Team, PPT are an integral part of the Bucks pathway supporting people who exhibit behaviour that challenges.

Main duties of the job

The post holder will provide long-term support for identified people with complex needs by identifying and responding to potential problems as they emerge, identifying early signs of placement breakdown and linking the person/service provider into appropriate support. PPT Co-ordinators are an integral part of the Learning Disability Health Team. The PPT Co-ordinator’s role will include:

Supporting people with a learning disability and challenging behaviour to live in welcoming and supportive communities.

Providing personalised, flexible and responsive support to assist individuals, families, services and communities to access accurate and timely information to clarify their goals, strengths and needs.

Supporting the delivery of PPT’s training suite and providing opportunities to embed the learning into the day to day.

Operating as a service coordinator (rather than a service provider) and helping the referred person and their families/carers to plan, select and receive needed supports and services

 Assisting people and their supports/services to manage barriers and constraints in achieving their preferred lifestyle, including:

assisting local services to act early before the barriers and constraints jeopardise the person’s ability to remain in their local community

Detailed job description and main responsibilities

To build and maintain effective working relationships with referred people, their carers/families and communities in a defined local area.

To support access to accurate, timely and relevant information and to assist individuals, families and communities to access information through a variety of means.

To provide individuals and families with support and practical assistance to clarify their goals, strengths and needs through the development of individual care and person-centred plans.

To support access to effective independent advocacy and self-advocacy support as required.

To assist individuals, carers and families to use personal and local community networks to develop practical solutions to meet their identified goals and needs.

To assists individuals and families to access the supports and services they need to achieve their identified care plan and goals, including access to local health and social services.

In collaboration with the Service User and their family, to monitor the quality of the service provided by the funded support services and raise concerns with the Learning Disability Health Team.


- To collaborate with the other members of Learning Disability Health Team to facilitate implementation of recommended intervention and support strategies.
- Where training and development needs are identified, supporting access to the Training Function within the Team.
- Ensuring effective risk assessments and risk support plans are in place to ensure that vulnerable people are kept safe.
- Facilitating the safeguarding process where needed

Knowledge

Essential criteria
  • Commitment to principles of social inclusion and demonstrated experience of working in a person centred way
  • Highly developed verbal and written communication and negotiation skills with the ability to build and nurture relationships, including the ability to advocate for others.
  • Ability to effectively manage a variety of tasks and achieve effective outcomes.
  • Competency in the use of computers and related technology.
Desirable criteria
  • Knowledge of legislation, theory and policy in relation to people with a learning disability.
  • Ability to develop plans and achieve positive outcomes for individuals within the context of organisational policy, procedures and resources.
