Job overview

We are currently seeking an experienced Registered Mental Nurse Band 6 to work within our Adult Community Mental Health Services (ACMHS) as a Front-Door Brief Assessor, via a Bank Contract, based in Borehamwood for our Adult Community Services Team. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.

 

This role is vital for providing timely, person-centred assessments for referrals through the Front-Door pathway, ensuring risk identification and compliance with Care Act and National Institute for Health and Care Excellence (NICE) standards.

 

HPFT provides services for service users with mental health, learning disability and children’s and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.

HPFT has a range of healthcare settings across the County and therefore access to a car would be beneficial unless you have a disability as defined by the Equality Act 2010 which prevents you from driving

 

All applicants need to hold a valid NMC Professional Registration and can evidence this

Main duties of the job

The post holder will complete structured brief assessments for service users referred to the service and identify and manage risks, including any safeguarding concerns.

 

This role is critical for meeting 28-day access targets, reducing clinical risk, and supporting patient safety. You will play a key part in improving service user experience and maintaining high standards of care.

Detailed job description and main responsibilities

·       Coordinate referrals and signposting to appropriate services.

·       Document assessments accurately in PARIS and liaise with multidisciplinary teams.

·       Participate in triage and review meetings to support safe and efficient service delivery.

·       Any other assignment that may be deem appropriate by the management.

  • Additional training in risk assessment or crisis intervention.

Experience

  • Proven experience in mental health assessments and risk management.

Knowledge & Skills

  • Ability to complete structured assessments and identify high-risk factors
  • Competence in electronic record systems (e.g. PARIS).
