Job overview

  • To work as part of the Placement Team which provides Health and Social Care placement services to the Trust.
  • To focus directly on the needs of Service Users using the Support, Time and Recovery Model to help them move through the health and social care pathway.
  • To work across service and/or care groups as necessary.
  • To work closely in association with the Service Manager, Placement Advisors, other senior staff, wider team and external agencies.
  • To work and focus on Recovery Principles.
  • To undertake reviews of service users in funded placements to feedback to the team in order to ensure placements are effective and providing safe and quality care.
  • To work with our Care Providers to maximise service user participation in their local community.
  • To undertake direct Service User work where appropriate, in order to proactively engage Service Users in working to achieve their desired outcomes; in line with recovery principles and agreed care plans.
  • To consider onward accommodation pathways out of placement in an enabling fashion.

Main duties of the job

  • To provide support and give time to an allocated group of Service Users and thus promote their recovery and promote their place in the broader community.
  • To assist Placement Advisors or Care Co-ordinator's to assess, plan, implement and evaluate individual Service Users recovery plans within their Placements.
  • To ensure that Service Users engage beneficially with the agreed Care Plan and access appropriate services provided on a regular and consistent basis.
  • To positively promote the pathway towards independent living of service users within community.
  • To offer practical support during the transitioning between placements and accommodation; including service user travel where appropriate.
  • To build relationships with the Local District Councils to form working relationships

  • IT literate and able to present information to a range of audiences through a range of media

Clinical Responsibility

  • To act as primary worker to cases allocated by the Team Manager to ensure service users’ progress, level of functioning and mental state and alert the appropriate staff involved in the service users’ care

Detailed job description and main responsibilities

STaR Worker

An opportunity has arisen for a STaR Worker to join the team at Hertfordshire Partnership Foundation University NHS Trust's Placement Service.

The post holder will work closely with Placement advisors; namely Nurses, Social Workers and Occupational Therapists, within the team and wider mental health services to support individuals through the social care pathway within placement settings.

The individual will be motivated, adaptable and flexible; to work with individuals in creative ways to support and enable individuals through the placement pathway alongside placement advisors. A supportive, pro-active, and enthusiastic approach to support colleagues, social care providers, service user's and their families is required in this role. The applicant will be well organized; and passionate about quality care; as well as promoting independence; and supporting the implementation of effective care and support plans alongside social care providers. 

Qualifications/Equivalent Experience Essential

  • NVQ Level 3 qualification or Mental Health Certificate Level 3 or agreement to undertake such training as soon as resources permit.
  • NVQ Mental Health Certificate Level 2.
  • Understanding of the Care Act
  • Understanding of the Mental Health Act- or ability to learn.
  • 2 years’ experience of working within the social care field, or similar or with personal lived experience of Mental Health difficulties.
  • Understanding of housing pathways within Hertfordshire
