Job overview

We are looking for two highly motivated and enthusiastic individuals to join our expanding Pharmacy and Medicines Optimisation team at HPFT, in newly created posts supporting community mental health services. These roles are part of the wider growth of the pharmacy team and offer an exciting opportunity to help shape and develop mental health clinical pharmacy services across our community teams in Hertfordshire.

This is a fantastic opportunity for pharmacists who want to develop and use their clinical skills and knowledge in a supportive environment, with a strong focus on supporting high risk medicines, medicines reconciliation and medication reviews. Mental health knowledge is essential, and will be supported to grow and develop in the role while working closely with the multidisciplinary team to influence and shape services for the future.

Main duties of the job

Clinical and professional practice- Provide a specialist clinical pharmacy service in the community setting, ensuring safe, effective and evidence-based use of medicines. Conduct comprehensive medication reviews, support high risk medicines optimisation, and promote adherence while working collaboratively within multidisciplinary teams.

Safety- Reduce medication-related risks by supporting safe practice through identifying, reporting and investigating incidents.

Leadership- plan and manage own workload, support pharmacy service modernisation, and contribute to the development of medicines optimisation initiatives in the community.

Education and training- Provide training and education that meet statutory, professional and regulatory requirements for pharmacists and technicians.

Detailed job description and main responsibilities

Essential Qualifications

  • Degree in pharmacy and registered with the General Pharmaceutical Council (GPhC).
  • Postgraduate clinical pharmacy qualification (or equivalent experience) with evidence of ongoing CPD.

Experience

  • Post-registration mental health clinical experience, including multidisciplinary working, clinical pharmacy provision and audit completion.
  • Experience in self and staff development.

Skills and knowledge

  • Strong organisational, planning and prioritisation skills, with experience of delivering training to healthcare professionals.
  • Excellent written and verbal communication skills, with sound knowledge of mental health medicines.

 

Additional information

  • Participation in late duty, weekend, bank holiday and oncall rotas may be required to support equitable seven day services.

  • Degree in Pharmacy- BPharm, MPharm, BSc Pharmacy
  • Registration with the General Pharmaceutical Council (GPhC)
  • Postgraduate clinical pharmacy certificate/ diploma (or working towards) or equivalent experience
  • Demonstrates own CPD
Desirable criteria
  • Registration with the Royal College of Pharmacy (RCPharm)
  • Postgraduate certificate/ diploma in Mental Health Pharmacy or equivalent experience
  • Postgraduate diploma in Advanced Clinical Practice
  • Management qualification
  • Leadership qualification
  • Independent prescriber or working towards

Experience

Essential criteria
  • Post-registration clinical knowledge and experience in mental health
  • Experience of working in a MDT
  • Evidence of involvement in clinical pharmacy provision

  • Good management skills- including recruitment, supervision, and appraisals
