Welwyn and Hatfield Integrated Care Team are winners of Hertfordshire Community Trust Leading Lights awards for Demonstrating Fantastic Patient Care and are looking to recruit band 5 Nurses to our award-winning team. Hertfordshire Community Trust has a Good CQC rating acquired in May 2020 and we are working towards achieving an Outstanding rating.

We deliver high quality nursing care across the Welwyn and Hatfield locality to prevent admission to hospital, facilitate discharge, deliver planned nursing intervention and manage long term conditions.  

The successful candidate would be supported to develop skills and join a team who are committed to demonstrating the trust values who care about each other as well as their patients.

We work collaboratively with our partnership organisations across East and North ICB, Hertfordshire Foundation Trust and Hertfordshire 

Main duties of the job

Our team is the Welwyn and Hatfield Locality.

The HCT Integrated Community Care Teams are committed to delivering high quality, evidence- based nursing care to patients within their own homes and offer an integrated multidisciplinary approach in helping to support patient discharge and prevention of admission to hospital.

We offer a comprehensive induction programme and the opportunity to work in a friendly, dynamic, professional and caring team.

Successful candidates will need to have an ability to creatively problem solve and work on their own initiative and will need to demonstrate an understanding of current professional and community issues. Community nursing experience is preferred but there will be opportunities for training and development for the right applicants.

A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
