Unscheduled Care Coordinator

We are looking for dynamic and motivated Unscheduled Care Coordinators to join our Out of Hours (OOH) operational team. This is a vital role in supporting the smooth delivery of urgent primary care services across multiple regions. Working flexibly, you will help manage clinician allocations, patient flow, and logistical coordination to ensure safe and timely patient care. Shifts are allocated in advance by the rota team based on service demand, offering a balance of flexibility and consistency for staff.

You will be responsible for coordinating clinicians, managing home visit allocations, monitoring vehicle movements, and ensuring that patients are seen promptly. The role involves real-time communication with drivers, clinicians, and reception teams to keep services running efficiently and safely. You will maintain accurate operational records, support the Shift Manager in managing service performance, and help ensure adherence to HUC’s policies and NHS standards. Flexibility is key, as shifts include weekday evenings, overnights, weekends, and bank holidays.

The Unscheduled Care Coordinator (Flexi) plays a central role in ensuring the effective running of HUC’s Out of Hours and urgent care services. Working closely with the Unscheduled Care Shift Manager, you will:

  • Allocate home visit requests to clinicians and coordinate patient flow across multiple sites.

  • Monitor and track vehicles, resources, and real-time operational activity using IT systems.

  • Act as the main communication link between clinicians, drivers, and administrative teams.

  • Maintain clear records, performance logs, and detailed shift handovers.

  • Uphold patient confidentiality, follow safeguarding procedures, and promote HUC’s values of respect, integrity, and teamwork.

  • Participate in mandatory training, service improvement initiatives, and reflective practice.

You will need strong organisational, communication, and problem-solving skills, as well as the ability to stay calm under pressure. The role is suited to individuals with operational or administrative experience in a healthcare, emergency, or logistics setting—or those with transferable experience and a willingness to learn.

  • Experience in an operational, administrative, or coordination role within a fast-paced environment where prioritisation and accuracy are critical.
  • Excellent written and verbal communication skills, with the ability to convey information clearly across multidisciplinary teams.
  • Proven understanding of resource management, scheduling, and patient flow within healthcare or service delivery, or relatable experience with a willingness to learn.
  • Proficiency in IT systems including Microsoft Office, call-handling, and operational tracking software.
  • Strong problem-solving and analytical skills with the ability to make sound, evidence-based decisions in real time.
  • High attention to detail and accuracy in recording and reporting operational data.
  • Ability to manage competing priorities while maintaining focus on safety and service delivery.
  • Demonstrated integrity, professionalism, and confidentiality when handling sensitive information.
  • Emotional resilience with the ability to manage stress and maintain performance under pressure.

  • Ability to mentor and support junior colleagues to promote teamwork and development.
  • Competence in using analytical tools or dashboards to support decision-making and service improvement.
  • A proactive approach to learning, self-improvement, and contributing to service development.
