Job overview

We have an exciting opportunity for a Mental Health Nurse to take up a full time position with the Community Mental Health Older Adult Services  in East Hertfordshire. As a multi-disciplinary team we provide holistic assessment and treatment pathways for older adults, and people with mental health and frailty needs. Our service provides seamless pathways between primary care and secondary care services for people with functional mental illness, and dementia.

We are looking for a dynamic Nurse who is passionate about the care of our older adult service users. We consider transferable skills from other areas of mental health services as valuable and a positive contribution to a well-rounded team.

We cover the East locality of Hertfordshire, and our bases are Oxford House, Bishops Stortford, and Rosanne House, Welwyn Garden City. This role is to cover the Welwyn Garden City and surrounding area locality, with flexibility to support other areas of the East locality if needed.

As a team, we can offer multi-disciplinary support from Clinical Psychologists, Consultant Psychiatrists, Occupational Therapists, Social Workers, as well as Mental Health Nurses. We also provide regular CPD sessions, reflective practice, staff support sessions, individual and group supervisions, and academic training, as well as access to external training resources.

Main duties of the job

As the band 6 Community Nurse, you will be expected to:

  • Take responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care and management of a defined caseload, including liaison with other agencies and, where appropriate, the supervision, development and teaching of staff and/or students.
  • Act as care co-ordinator to a case load of service users, ensuring the care plan is met at all times.
  • Work as a member of the multi-disciplinary team.

All staff should comply with the Trust’s Anti-Discriminatory Statement, Employee Charter, Trust Policies and Procedures, Code of Conduct and Equality and Diversity.
