The North East Kent Home Treatment Team is looking for a Registered Mental Health Nurse to join their team.
The community mental health nurse will carry a defined caseload of patients who have memory impairment and/or functional mental ill health.
The community mental health nurse is a key member of the multidisciplinary team and will have a continuing responsibility to assess, plan, deliver and evaluate care, communicate findings, influence change and promote health and best practice, patient safety and experience.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
To help manage a defined caseload of people who have functional and organic mental illness under supervision.
Carry out comprehensive holistic assessments which take account of relevant mental, physical, social, cultural, psychological, spiritual and environmental factors, in partnership with the patient and others through interaction, observation and measurement.
Work collaboratively with other disciplines, patients and their carers to agree a holistic person centred care plan that addresses the needs identified through assessment.
To deliver and evaluate safe, competent, person-centred care in partnership with the service user and their carers to support recovery.

Qualifications
Essential criteria
- A recognised professional qualification as a Registered Nurse (Mental Health) and NMC registration, or equivalent qualification for Occupational Therapy.
Desirable criteria
- Practice Educator/Mentor Qualification
Knowledge and Experience
Essential criteria
- Experience of acute mental health care.
- Experience of nursing a range of mental health conditions.
Desirable criteria
- Experience of individual & group work
Skills
Essential criteria
- The ability to travel across a large geographical area 24 hours a day.
- knowledge of current best practice in providing holistic interventions to people experiencing acute episode of mental ill health.
- knowledge of and ability to apply relevant legislative frameworks and policies relating to care e.g. Mental Capacity Act, Safeguarding, Consent to Treatment.
- A robust understanding and ability to apply principles of risk assessment and risk management.
- Understanding of the impact of acute episode of mental illness on function and occupation of clients, family and carers.
