Clinical Nurse Specialist

We are looking for a Band 7 Clinical Nurse Specialist to join our Community Nursing Team based in Faversham. 

Our Community Nursing Teams provide interventions for housebound patients aged 18 and over delivering personalised nursing care to those with complex and long-term conditions. The aim of our service is to enable patients to stay safe and well in their chosen environment.

The service actively works to prevent avoidable admissions by providing tailored care that meets the patient need and assists them in actively achieving their long-term health goals and self-care where possible. 

Prif ddyletswyddau'r swydd

As the Clinical Nurse Specialist you will be responsible for the management and leadership of individual staff members and the quality of the care they deliver. You will represent community nursing across the Primary Care Network. 

You'll need to be an experienced Community Nurse who displays sound clinical judgment and can act as a role model for the team.  

You'll support and develop junior members of your team and ensure the standards of care delivery remain high.

Qualifications

  • Registered health care professional (NMC registration) V300 Non-medical prescriber or a willingness to complete within an agreed time frame Health related degree and evidence of relevant post graduate development at Level 6 Evidence of Post Graduate Specialist qualifications relevant to area of practice; specialist practitioner district nursing, non-medical prescribing, mentorship, coaching, and clinical expertise Management or supervisory qualification / programme of study

  • Preparing to Teach in the Lifelong Learning Sector / Diploma in Teaching in the Lifelong Learning Sector, Post Graduate Certificate of Education or Certificate in Education Working towards Master level study

Experience

  • Minimum of five years’ experience in area of practice Experience of leading and managing teams Experience of developing policy and service development plans with practical experience of change management. Practical experience in managing health related projects/complex caseloads Supporting others (mentor / educator / supervisor) experience Experience with the use of Microsoft Office

Knowledge

  • Understanding of National Service Frameworks and NICE guidance to relevant to care setting and its application across health care services Evidence of continued relevant professional development and implementation in practice within the last 12 months Proven ability to develop effective partnerships to achieve practical outcomes across organisational boundaries including statutory organisations, external agencies, community and voluntary organisations Supporting others (mentor / educator / supervisor) experience
