Main duties of the job

The successful post holder will be part of a team of rotational pharmacists in training posts which provide a comprehensive and high quality pharmaceutical service to distinct areas of practice.

These include aseptic, clinical pharmacy services such as medicine and surgery, and the dispensary.

Detailed job description and main responsibilities

As a Rotational Pharmacist you will be expected to promote the safe, rational and cost effective use of medicines in accordance with the Drugs and Therapeutics Committee recommendations and other evidence-based guidelines.

You will be supported through an education and development programme and given the opportunity to undertake the postgraduate Diploma in General Pharmacy Practice.

You will be expected to provide a pharmacy service in accordance with current professional standards and clinical governance guidance.  To develop the required knowledge and skills whilst progressing through the rotations.

Medicine reconciliation on admission and discharge.

Clinical Pharmacy Services

To provide a clinical pharmacy service to Clinical Divisions as part of ward-based pharmacy teams.

To promote the safe, rational and cost effective use of medicines in accordance with the Drugs and Therapeutics Committee recommendations and other evidence-based guidelines.

To evaluate the clinical appropriateness, safety and legality of prescriptions based upon the patient’s diseases, co-morbidities and other medication and treatments taken, contacting the prescriber to make agreed changes as appropriate.

To take part in medicines reconciliation on admission

To promote efficient discharge planning inclusive of patient counselling and education

To liaise with other pharmacists, doctors and GPs to ensure efficient transfer of clinical and medicines management issues

To participate in adverse drug reaction and intervention monitoring and clinical risk management

To follow procedures to reduce medicine waste (e.g. by using patient own drugs)

To ensure efficient supply systems (e.g. one-stop dispensing), reducing the risk of critical missed and delayed doses by working with technicians and assistants and dispensary.

To work with the ward nurses, dispensary and pharmacy ward team to improve patient flow.

To attend consultant ward rounds as deemed appropriate by the Lead Pharmacist for Clinical Services.

To answer or refer Medicines Information queries as appropriate.

Patient Services

To work in the dispensary checking prescriptions for inpatients and outpatients in accordance with the Dispensing and Checking procedures

To prioritise work to deliver an efficient service

To provide patients with adequate information and advice on how to use their medicines effectively in accordance with the Patient Counselling procedure

Aseptic Services

To carry out all aseptic duties according to national standards, relevant legislation and departmental SOPs

After suitable training the post holder will responsible for the release of products prepared in the Aseptic Unit for use within the trust, as specified under section 10 exemption of the Medicine Act 1968.

Education and Training

Participation in a Postgraduate Diploma in General Pharmacy Practice is expected as part of the Foundation Pharmacist’s personal development programme.

To participate in the education and training of pharmacy and other healthcare staff and pharmacy students to graduate level

To undertake Clinical Audit work within Pharmacy and in conjunction with others

To contribute to the implementation of pharmacy issues arising from audit projects

To maintain an active portfolio of continuing professional development (CPD)

To participate in the departmental CPD educational sessions

Qualification

Essential criteria
  • Professional knowledge acquired through vocational masters degree in pharmacy, foundation training and experience.
  • Registered with the General Pharmaceutical Council
