Job overview
We have an opportunity for a Band 7 Complex Care Nurse to join our Complex Care Nursing service in Maidstone.
Our complex care nursing service provides proactive and responsive care delivery to support patients with frailty and other complex long-term conditions, such as Heart Failure COPD and diabetes.
We support patients with robust management plans to promote self-management, where possible, and optimise long term condition management and preventing unplanned hospital admissions.
Main duties of the job
Each nurse in our team is responsible for the management and nursing care delivery to a defined patient caseload within an aligned geographical area.
Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from acute hospital, be terminally ill or have physical disabilities.
As a Band 7 Complex Care Nurse you will work as part of a team to keep people safe and well in Maidstone. You will use your clinical judgement to support evidence-based care planning and make sure that people who use our services have a good experience.

- Registered health care professional (NMC and experience in relevant healthcare setting)
- Evidence of Post Graduate Specialist qualifications relevant to area of practice ( examples –:non-medical prescribing; mentorship , coaching , clinical expertise)
