Senior Nurse Assessor Children and Young People

The Senior Nurse Assessor plays a vital role in improving the lives and outcomes of Children and Young People (CYP) in Kent and Medway living with physical and/or mental health conditions.
Working within the Children and Young People’s (CYP) Continuing Care Service, the post holder will assess, review, and monitor care packages, and placements for CYP referred to All Age Continuing Care (AACC) and Specialist Placements.
Commissioned by the Kent and Medway Integrated Care Board (ICB), this role supports the delivery of high-quality, evidence-based services that minimises the impact of illness and promote holistic well-being across physical, mental, and emotional health domains.

You will conduct evidence-based assessments to determine eligibility for NHS Continuing Care, presenting findings to a multidisciplinary panel. They must demonstrate strong clinical assessment and analytical skills and be confident in making clinical decisions.
The role involves collaboration with a wide range of services, including local authorities, education and social care teams, children’s health and disability services, hospitals, hospices, and adult Continuing Health Care.
Travel across Kent and Medway, including out-of-area placements and hospitals, is required.
Flexible working is supported through multiple office locations and a hybrid model of office and home-based work. The post holder will also provide clinical support to team members.
