Job overview

The successful candidate will work as part of an integrated service in a multi-disciplinary team of Therapists, Nurses, Wellbeing Support Workers GPs and Social Care Workers to deliver quality integrated care to patients in the community.

You will be supported with your own caseload to provide assessments and interventions to patients in their own homes and other community clinical settings. You will take part in regular in-service training and supervision. You will be trained to use digital technology as well as face to face sessions to meet the demands of the service. Training and learning opportunities are abundant. 

 

Main duties of the job

Main duties are to work as part of the MDT to provide assessments and treatments to the adult population who suffer with a variety of health conditions. Following own best clinical practice and the guidelines of team and trust to empower patients to support improve their own health care needs.

Detailed job description and main responsibilities

  • To be professionally and legally accountable for all aspects of own work, including the management of patients in your care.
  • To be responsible for maintaining own competency to practice through CPD activities, and maintain a portfolio which reflects personal development.
  • To undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies; using advanced clinical reasoning skills and manual assessment techniques to provide a clinically reasoned working diagnosis of their condition.
  • Formulate and deliver an individual physiotherapy treatment programme based on a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills e.g. manual physiotherapy techniques, patient education, exercise classes, electrotherapy techniques and other alternative options.
  • To take delegated responsibility from the senior physiotherapist for managing patients with particular conditions and be responsible for providing physiotherapy assessment and treatment plans for patients with these conditions safely.
  • Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
  • Use a range of verbal and non-verbal communication tools and channels to communicate effectively with patients to progress rehabilitation and patient management. This will include patients who may have difficulties in communication, For example, patients may be dysphasic, depressed, deaf, blind, linguistic barriers or may be unable to accept diagnosis.
  • Evaluate patient progress, reassess and alter treatment programmes if required.
  • To be responsible for the safe and competent use of all equipment used.
  • To manage clinical risks within own patient caseload and to work within trust guidelines. 
  • To be responsible for maintaining accurate and comprehensive patient treatment records in line with HCPC & Chartered Society of Physiotherapy (CSP) standards of practice.
  • Represent physiotherapy service and / or individual patients at the multi-disciplinary team meetings, to ensure the delivery of a co-ordinated multidisciplinary service, and integrate physiotherapy treatment into the treatment programme. This will include discussion of patient care, patient progress and involvement in discharge planning.
  • Keep up to date with trust and departmental mandatory training.
  • Maintain up to date continuous professional development.

  • Diploma/degree in Physiotherapy
  • HCPC registration

Knowledge and expertise

  • CPD diary including evidence of reflective practice and undergraduate experiences.
  • Theoretical and clinical knowledge underpinning physiotherapy practice.

Experience

  • Broad range acquired at undergraduate/newly qualified level.
