Detailed job description and main responsibilities

As a Band 5 you will you will provide direct clinical care to surgical patients. Working alongside the nursing team, MDT and medical staff you will provide evidence based care. 

Complete observations, escalating any concerns appropriately to the nurse in charge and medical teams. Providing wound care, preparing patients for theatre, admitting and discharging patients safely.

Be able to prioritise your workload whilst acting as an advocate for your patient, assessing and implementing their individual plan of care with accurate and concise documentation.

Be a clear communicator within your team and the wider service and act in a sensitive and professional manner at all times, promoting the Values of the Trust.

You will be trained to perform venepuncture and cannulation and IV drug administration, wound care and stoma care.

2. a) Assist trained staff to carry out nursing care ensuring the patients’ confidentiality and privacy are maintained.

3. a) Ensure patients’ safety, comfort, and welfare at all times.
4. b) Report accidents, incidents, losses as they occur to the nurse in charge.
5. c) Attend yearly up-dates on all statutory training, including basic life support and manual handling and moving.
6. c) The post holder must comply with all relevant policies, procedures and training on infection prevention and control.

Minimise the Risks Arising from Health Emergencies

4. a) Report untoward incidents and occurrences regarding changes in patient’s condition to the nurse in charge/named nurse as they arise.
5. b) Assist in emergency care once training undertaken.

5. a) Liaise with nursing, medical, and other disciplines within our defined clinical area and other departments and hospitals.
6. d) Be supportive to the patient and their family/carers at all times.

Receive and Transmit Information

6. a) Ensure teamwork reports are completed and details entered onto the computer.
7. b) Admit and discharge patients via the computer.
8. c) Arrange waiting list admissions and participate in transfer and discharge arrangements for patients.

Store and Retrieve Records

7. a) Maintain accurate registers and bed states.
8. b) Ensure patients’ notes are filed and up to date.
9. c) Ensure pathology results/X-Rays/reports are available and filed as necessary

7. a) Anticipate and act on the physical and psychological needs of the patient and meet the same.
8. b) Provide a relaxed and professional environment conducive to promoting the well-being of the patient

7. a) Provide effective working relationships with patients/relatives/carers/ nursing team/medical and multi-disciplinary teams.
8. b) Report any complaints to the nurse in charge immediately.

Develop in the Work Role

10. a) Attend in-service training, study days, seminars, courses as rostered.
11. b) Attend a yearly IPR/Appraisal to set objectives and Personal Development Plan for the following twelve months with clinical leader.
12. c) Attend / undertake leadership and management courses.
