Health Improvement Learning Disability Nurse

NHS AfC: Band 7

This is an exciting opportunity for an experienced learning disability nurse to lead a project aimed at improving the health and wellbeing of adults in Bolton who have a learning disability.

The successful applicant will work alongside the existing, multi-disciplinary community learning disability team, our colleagues in primary and secondary health services and people with a learning disability, their carers and advocates  to lead on a number of initiatives aimed at improving access to appropriate health care.

The post holder will be required to lead on the promotion and development of educative interventions to be delivered to individuals with a learning disability, carers, provider services and healthcare colleagues to promote the identification of poor health indicators and increase the uptake of local and national screening projects.

The post holder will be required to contribute to the development of an LD conference which will have a good health agenda and which aims to address health inequalities subsequently reducing the risk of premature deaths.   The role will also involve direct work with specialist services to consider key health conditions and address barriers to healthcare.

There will be a requirement to lead on audit of annual learning disability health screening to help to evidence a link between health screening and improved health outcomes.

The post holder will work alongside the local learning disability mortality review (LeDeR) team to contribute to the local review process and learning from reviews action plan.

The successful applicant will be an experienced learning disability nurse who can evidence excellent time management skills, previous management of project work and up to date knowledge of the health inequalities experienced by people with a learning disability.   The successful applicant will be required to demonstrate excellent communication skills, an ability to participate in team working alongside an ability to work independently and proactively.

This post is a result of a successful funding bid to progress learning gained from completed  learning disability mortality reviews.  This is a part time, fixed term position for 12 months.
