An opportunity has arisen in the department for a band 4 Dietetic Assistant Practitioner.  

This would suit a Dietetic Assistant practitioner wanting to develop skills in a new team or as a training opportunity for those who hold a nutrition degree and wish to become a Dietetic Assistant Practitioner.

The post is based with the community nutritional support Dietitians at Brierfield Health Centre but carries a domiciliary caseload so the post holder would be expected to work  in community localities across East Lancashire.  It would therefore suit a car driver/owner.  

Working hours are Monday to Friday 8.30am till 4.30pm.  

Main duties of the job

Supporting adult patients in the community with malnutrition.  The majority of these patients will be elderly and frail and in a Care Home setting.  

To work as a member of team assisting in the delivery of services supporting registered Dietitians in assessing and treating patients and supporting their carers/families. 

This may include gathering information for assessments, implementing treatment programmes, and making recommendations for dietary changes and oral nutritional supplements when required.  

Supporting Care homes with nutrition training.

The East Lancashire Hospitals Trust Dietetic Department is one of the largest in the region and covers the hospitals and communities of East Lancashire.

The department covers both inpatients, outpatients and the community and looks after patients from from cradle to the grave. 

The department has nearly 80 staff and covers many specialties and is known for its innovation, development and training.    

The department support the clinical training of dietetic students, medical students and other health care professionals as well as supporting T-level students on health placements.  The post holder would be expected to support the department with this role especially in regards to Dietetic students and T-Level students.  

Detailed job description and main responsibilities

All Patient records are electronic whether it be Cerner as an inpatient or EMIS in the community.   Good It skills are therefore required.  

The post holder will also be expected to:

- use existing clinical pathways to assess, set patient centred goals, create treatment plan, monitor and evaluate a designated caseload of patients under the supervision of a Dietitian.  

- provide clinical care to a caseload of patients as specified by the Dietitian /Dietetic service working within the limits of your competencies under regular supervision in a variety of setting including individually or in groups. 

- advise, support and educate patients and carers in the self-management of the patient’s condition.  The post holder will be required to communicate effectively with a range of healthcare professionals, patients,carers and family members.  

- liaise effectively with other departments/services /agencies, including information relating to patient interventions and service delivery. 

- follow agreed clinical pathways and interventions and escalate to Dietitian / supervisor when variations are noted or required.  

- make decisions within the scope of competencies and following clinical pathways. 

Workload will be delegated by Dietetic Clinical Leads based on competencies.
