Band 4 Registered Nursing Associate – Homeless Health

An exciting opportunity has arisen for a unique band 4 Registered Nursing Associate working in Homeless Health Outreach in Blackburn with Darwen. We welcome applicants who are passionate and committed to working with individuals who are at risk of sleeping rough or experiencing homelessness. This is a great opportunity to work in a multi-agency partnership and develop close working relationship with a supportive Band 6 Senior Staff Nurse.

The successful candidate will work from satellite spaces and connect within community venues (e.g. GP surgeries, hostels, pharmacies) where existing and potential service users live or are based. The Registered Nursing Associate will work closely with the Senior Staff Nurse and support with any continuous treatment as documented within care plans. The Registered Nursing Associate will work within their scope of practice and competence and you will receive regular management and clinical supervision by the Senior Staff Nurse throughout your secondment.

This unique opportunity is for 12 months. This post holder will work Monday to Friday 08:00 – 16:00hrs or 09:00 – 17:00hrs.

This candidate will work closely with a multi-disciplinary team consisting of a Housing Outreach Team, Vulnerable Person’s Case Co-ordinator (Calico), Engagement Worker and Lived Experience Worker (ELHT Alcohol Care Team). You will engage with Service Users and support them to access the right care to meet their presenting needs to ensure the best possible outcomes for the individual.

This outreach role will adopt making every contact count and they will support identify at risk individuals. This multi-agency approach will enable collaborative working across health, social care and voluntary sectors and promote retention and reconnection with services, focusing on reducing barriers to engagement and re-engagement helping individuals’ problem solve through life challenges they are experiencing.

They will provide safe, personal and effective care and support to patients, families and service users working within a multidisciplinary team and carrying out a diverse range of duties. 

They will support the registered nurses, contribute to ongoing assessment and care planning for patients.

They will monitor the condition and health needs of patients on a continual basis in partnership with the multi-professional team.

 They will receive regular support and supervision from a Registered Nurse and be encouraged to take personal responsibility for clinical supervision and continued professional development plan.

Please note: Work is ongoing to transform adult physical health services across the Pennine Lancashire Footprint supported by commissioners within the Lancashire and South Cumbria system. This transformation aims to ensure service users in Blackburn with Darwen and East Lancashire receive equitable service provision from one service provider. From around 1st July 2024, the service provider will be East Lancashire Hospitals Trust. Staff employed by LSCFT will therefore transfer from LSCFT to East Lancashire Hospitals Trust around 1st July 2024 as part of this wider transformation plan.  

 

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
