Job overview

Clinic Based Services are looking to employ a motivated and enthusiastic Specialist Physiotherapist to join the Community Rehabilitation Team. 

The post holder will be responsible for the assessment, treatment, planning, delivery and evaluation of evidence based highly specialised AHP interventions within the areas of:

Community Rehabilitation Services which could include Intermediate Care, Falls Service and/or Domiciliary Physiotherapy Service dependent on area of need and expertise.

The successful candidate will have evidence of AHP clinical experience equivalent to master’s level in a range of clinical settings including multi-disciplinary team /multi-agency collaborative working, proven experience of working with complex cases, and experience of working autonomously and managing own caseload.

Main duties of the job

The post holder will:

Work as an autonomous advanced practitioner to provide a high standard of assessment, diagnosis, treatment and highly specialist expert advice to patients, carers and staff

Develop and promote a holistic, supportive and patient centred service that facilitates education, acceptance and lifestyle changes, which promotes self-management and aids recovery/rehabilitation

Act as a resource person for patients, carers and other healthcare professionals and participate in the delivery of specialist education and training across the service area

To act as a lead expert for rehabilitation and intermediate care  

Promote partnership working to improve outcomes for patients and facilitate seamless transitions in care through primary, secondary and tertiary services

Lead service development in community rehabilitation and intermediate care

Qualifications

Essential criteria
  • Degree or equivalent in Physiotherapy
  • Current professional registration with HCPC and responsibility for maintaining this registration
  • Masters module/post grad qualification in relevant field of clinical expertise (or working towards) / post-grad qualification/equivalent knowledge & experience to masters level

Knowledge

Essential criteria
  • Understanding of clinical governance framework and implications for practice including audit and research governance.
  • In-depth understanding of current NHS/government policy/Public Health/NICE guidelines on organisational change and the ability to adapt these appropriately to local service needs through publication of documents
  • Knowledge and appreciation of lone working risks – policy and procedure.
