We have vacancies in our Specialist Medicine team to provide expertise and Leadership in Geriatrics and a site lead for our Medical inpatient and Day Case services at LGH.

We are seeking experienced and enthusiastic pharmacists to join our Emergency and Specialist Medicine (ESM) Pharmacy team at University Hospitals of Leicester NHS Trust.   As one of our Advanced Specialist Pharmacists you will work alongside the MDT to support and develop our services within a speciality and collaboratively across the Emergency and Specialist Medicine service. Our service is well established and forward thinking. You will help us in our aim to improve the experience and care of our patients on specialist wards, non-specialist wards, day case and out-patients.

Our geriatrics role will involve working alongside our current Advanced Specialist Pharmacists in Geriatrics and Frailty Emergency Service (FES) you will be involved in overseeing pharmacists and technicians who rotate through the speciality whilst working with the wider service on other exciting initiatives.   There will be great opportunities to work with the consultant geriatricians on initiatives such as de-prescribing and falls prevention in order to improve outcomes for frail, older patients.  You will also have the opportunity to participate in our FES in-reach service within our ED. With a strong emphasis on teaching, you will be involved in carrying out innovative geriatric-related training for pharmacists as well as the wider multi-disciplinary team. 

At the Leicester General Hospital (LGH) site, the role will with have a particular focus on the Medical Day Case Unit (MDCU) process redesign and quality improvement. As the site lead for the LGH Pharmacy team, you will be a key member of the wider Pharmacy team and will co-ordinate the pharmacy team at LGH across specialities.  ESM Wards at the LGH currently include general and geriatric medicine, Brain Injuries Unit and Rehab.  Our MDCU provides services to neurology, gastroenterology, rheumatology and immunology amongst others.

You will need to be well-organised, pro-active, highly-motived and adaptable. Working closely with CMG Lead Pharmacists across multiple services you will provide leadership, training and development of pharmacy staff, trainees and students across ESM.

We have an ever growing number of Independent Prescribers and there is a strong desire to increase these numbers further so that responsibility for prescribing becomes further integral to our pharmacist's roles.

Main duties of the job

We are seeking experienced, highly motivated and forward thinking people for these varied roles. You role will involve:

· Providing specialist pharmacy and medicines optimisation to your specialist service (Geriatrics and/or LGH site lead)

· Managing the pharmacy service to one of our in-patient ward clusters, out-patient services and our Medical Day Case Unit (LGH site lead)

· Working with existing Geriatrics and Frailty leads to provide expert input into the care of patients and developing pharmacy roles within these services (Geriatrics lead).

· Acting as the daily Senior Pharmacist for ESM and/or acting as Site Lead Pharmacist for LGH

· Developing, implementing and monitoring guidelines and polices relevant to the service

· Teaching and training within the Department

· Providing input into MDTs within your area and identifying and promoting best practice

· Providing support, management and development to other team members

· Prescribing is a core aspect of our service delivery, and training will be provided to support this

Posts will based across the 3 sites of UHL, focusing currently at the LRI and LGH.

The post holder will be expected to contribute to departmental evening, out of hours and weekend duties as per current rotas

Detailed job description and main responsibilities

  • Maintain and develop an expert clinical practice in area of Specialty.
  • To lead and develop the specialist clinical pharmacy service to the specialist area in line with local and national standards and policies, ensuring the safe, clinical and cost effective use of medicines within UHL and the wider Health Community.
  • To deliver highly specialised pharmaceutical care to patients and to support other healthcare staff in meeting risk management, financial management and clinical governance responsibilities around the use of medicines.
  • To evaluate errors made with high risk medicines used by Specialty, to communicate these errors with relevant teams and to help reduce future occurrence of such errors throughout the Trust and wider Healthcare Community.
  • To undertake clinical supervision and mentoring of junior pharmacists, medicines management technicians and trainee pharmacists.
  • To be a source of expert specialist advice for other pharmacists, patients and all other healthcare professionals within the speciality, including participation in multidisciplinary and governance meetings.
  • To lead and develop clinical pharmacy audit and practice research within the specialist area, to act as a role model and national pharmacist expert in the Specialty area, networking with peers in other organisation.

Essential criteria
  • GPhC register pharmacist with relevant experience as per JD
  • Diploma or demonstrated equivalent clinical knowledge and experience
Desirable criteria
  • Pharmacist independent Prescribing

Skills

Essential criteria
  • Excellent written / oral communication skills
  • Ability to influence and negotiate with relevant senior clinicians and managers
  • Ability to demonstrate achievement of set goals

Experience

Essential criteria
  • Significant recent post-registration experience in hospital pharmacy
  • Medical / nursing / pharmacy staff teaching or training
