Job overview
Develop and maintain effective communications systems via the Transfer of Care Hub to ensure that information on capacity is kept up to date and used to inform decision making. This data is likely to be highly complex, sensitive and contentious at times so sensitivity and sharing of information in a timely way will be critical, ensuring that organisational staff and system wide staff are informed in a co-ordinated approach. Examples of this are daily on call system calls which are highly political in nature.
Represent the trust at internal and external meetings
Report to the Head of Group Flow and Clinical Service Leads on a regular basis regarding key performance indicators
Act as an ambassador for the Trust in professional and public settings, with patients, relatives, visitors and staff.
Main duties of the job
To co-ordinate the use of resources throughout the System over a 24-hour period, 7 days a week
Lead and support the Transfer of Care Hubcentre staff with managing complex and sensitive situations and supporting staff and patients across the site. Manages staff, including recruitment, appraisal, CPD, performance management.
Dial in to system teleconferences, acting as TOCH point of contact, to represent and contribute to the discussion about current position and challenges, providing decision regarding appropriate actions that can be taken at that time. Autonomous decision making, with a commitment to act and have a level of permission to ensure this role is credible in the wider system.
Responsibility to act, following up and resolving issues associated with staffing if not achieved locally, including escalating relevant information.
Support patient flow team, monitoring activity and understanding of current position and potential changes to this, including external impact factors and actions that may be needed to remove blocks, supporting escalation for any referral issues.
Responsible for development and delivery of effective site management to meet agreed local and national performance targets and standards, within budget and resources available. Monitor Length of Stay and delays, ensuring all action is taken to support flow and encourage HOMEFIRST
The Operations Manager will act as Silver Commander in the event of a major/significant incident.

Qualifications
Essential criteria
- Educated to 1st degree or equivalent in a relevant field
- Teaching and assessing/mentorship course or equivalent
- Evidence of continuous professional development
Desirable criteria
- Major Incident training/silver command
- Experience of working with patient flow or discharge
- Experience of working as an on-call manager- silver commander in the event of an emergency situation
Skills/Competencies
Essential criteria
- Experience of leading care (team of service level)
- Specialist Community knowledge qualification or relevant experience
- Knowledge of care delivery across both health and social care
- Specialist experience and knowledge of managing patient flow
- Ability to work with systems and processes to monitor and analyse data, e.g. Cayder
Desirable criteria
- Experience of continuing health care
- Experience of working across health and social care in partnership and knowledge of present health and social care agendas
- Experience of managing a budget/ production of QIPP targets
- Ability to develop and manage systems to support efficiencies
