We are looking for an Advanced Clinical Pracitioner to join our forward thinking and supportive team at Cauldwell Medical Centre (CMC) where you will be provided with an exciting opportunity to practice and further develop your clinical skills.
Main duties of the job

Main duties at Cauldwell Medical Centre include patient triage, home and nursing home visits, chronic disease management, acute assessments, and health promotion. The ACP supports QOF targets, reduces hospital admissions, and mentors nursing staff. They contribute to safeguarding, clinical supervision, and continuous service improvement, ensuring timely, safe, and holistic care across CMC.

Detailed job description and main responsibilities
- The post holder will be an experienced nurse or allied health professional, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
- They will demonstrate safe, clinical decision-making and expert care for patients within the general practice.
- The post holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
- The post holder will build effective working relationships with other health care professionals.

- Completed or working towards MSc ACP
- The post holder will work within the multi-disciplinary team to deliver excellent primary health care services within the practice.

Education/Qualification/Training
Essential criteria
- Professional Registration
- Post Graduate Qualification in specific specialty or equivalent experience
- Advanced clinical Practitioner Master’s Degree
- Independent Prescriber
- Evidence of continuous learning and professional development
Desirable criteria
- • Teaching and Mentorship (ENB 998 or equivalent)
Experience
Essential criteria
- Significant recent experience in General Practice and primary care
- Able to manage acute illnesses, long term conditions and minor ailments
- Experienced and competencies in the management of Long term Conditions
- Experience of working in a multidisciplinary way
Desirable criteria
- Experience of leading on the delivery of QOF in General Practice
Knowledge and skills
Essential criteria
- Specific knowledge of pertinent issues for Primary Care
- Experience of Working with Homeless People / People with Challenging Behaviours
- Advanced clinical skills and management of patients with complex issues
- Excellent verbal and written communication skills with team members, patients, carers, and other healthcare professionals whilst recognising people’s needs for alternative methods of communication
Desirable criteria
- Understands legislation and government initiatives particularly in relation to Primary Healthcare and Nursing

- Experience of using EMIS Web
