Bexley Clinical Care Professional Lead for Community Based Care

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

The primary responsibility of the Community Based Care CCPL will be to provide strategic leadership, operational insight and clinical input to ensure integration of primary, community and urgent care ecosystem to meet the ever-growing need within our Bexley population. This is a significant leadership role, being a key clinical representative within our partnership and voice in translating the experiences of patients, to meaningful system wide changes in transforming the front door into primary and community-based healthcare. The individual will be adept at building credible relationships that enable the trust and confidence of primary and community-based care colleagues across the borough, driving forward more integrated service delivery models for routine, urgent and same day access care.

The post holder will be responsible for facilitating the development of a clinical vision for community-based care in Bexley, making the Fuller vision a reality in Bexley through better joining up care and improving population health outcomes. The lead will also have a responsibility for collaborating and networking all the Clinical Care Professional Leads within the borough, so they are best supported to maximise their impact and form effective networks with their counterparts across the SEL integrated care system. This role will represent Bexley at a South East London level as well as on a number of key strategic meetings such as the Health and Wellbeing Board, the Bexley Wellbeing Partnership and the Community Based Care programme Board.

We are therefore seeking a clinical professional who is currently working in a front-line, patient facing role in the Bexley Healthcare System who is dedicated to taking on this leadership role 3 sessions per week on a fixed term contract of employment or secondment.  This individual needs to be passionate about improving patient care through working collaboratively with other service provider partners to make the strategic ambitions of more integrated care, an operational reality in Bexley.

  • Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area

Experience

  • An in-depth understanding of health and care provision and awareness of current ICS/system policies and priorities
  • Champions the principles of equality, diversity and inclusion both within and outside of the organisation. Provide effective, inclusive and compassionate leadership ensuring all voices are heard and creating and nurturing a culture where staff feel safe to speak up.
