Job overview

An exciting opportunity has come up to work as Band 5 Physiotherapist for Camden Integrated Integrated Community Health. 

Staff will benefit from an opportunity to develop knowledge and skills from other senior and specialist clinicians including advance clinical practitioners and first contact practitioners.  The service is also committed in staff development by providing regular supervisions and CPD opportunities.

JOB SUMMARY: 

  • You will be part of a community team where your proven ability to make high level clinical decisions will aid patient recovery. This will include assessing and  setting goals in partnership with patients, designing intervention plans, delegating appropriately to rehabilitation assistants, planning effective and timely discharge from the service,  and liaising with GP or other health care professionals. 
  • You will provide physiotherapy assessments, interventions and goal-based rehabilitation.  Following the required induction and support, you will be working autonomously providing intervention as well as co-ordinating complex inter-professional, multi-agency joint health and social care interventions.

Main duties of the job

  • To undertake comprehensive assessment of patients including those with diverse or complex presentations in patient's homes and/or clinic settings. 
  • To use clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition.
  • Formulate and deliver an individual physiotherapy treatment programme based on evidence based practice and treatment options using clinical assessment, reasoning skills and a broad knowledge and understanding of treatment skills.
  • To carry out assessment and treatments of a range of musculoskeletal, medical and neurological conditions with moderate physical effort on a daily basis.
  • To have basic knowledge assessment and management of long term conditions.
  • To manage a caseload of clients with simple to complex needs with supervision and guidance as necessary from senior staff. To successfully monitor, evaluate and review physiotherapy treatment/interventions within a multi-disciplinary framework and modify as necessary, taking a key worker role as indicated.

This role is a community physiotherapy role in Camden in CNWL NHS Trust.

Detailed job description and main responsibilities

·     To undertake specialist physiotherapy assessment of clients with diverse needs through subjective questioning, objective assessment and other written material available.

·     To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, clients prognosis and other physical, social emotional and mental health issues.

·     To set goals for treatment with clients and/or their carer and ensure that care is client centred and agreed by the client and/or their carer.

·     To set and maintain a high standard of clinical practice which is evidence based where possible, using standardised assessment and outcome measures, where appropriate

·    To independently triage/screen complex multi-disciplinary referrals from a variety of sources. This screening requires individual responsible action when at times no other professional is currently involved. This screening will establish priority, determine needs and initiate a plan of intervention.

·    To manage a caseload of clients with simple to complex needs with supervision and guidance as necessary from senior staff.  To successfully monitor, evaluate and review physiotherapy treatment/interventions within a multi-disciplinary framework and modify as necessary, taking a key worker role as indicated.

·      To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care.

·     To have the ability, knowledge and skills required for moving and handling people in community environments.

·      To ensure client and service records are recorded and maintained on IT databases and that client documentation is according to trust and local policies.

·      To prescribe, delegate and monitor the work of rehabilitation assistants and counter sign documentation as required.

·      To educate and teach the relevant carers/health workers to participate in on-going rehabilitation care programmes for clients, as appropriate.

·      To manage clinical risk in own caseload and ensure that rehabilitation assistants and students comply e.g. manual handling risk assessment and local policy regarding lone working.

·      Escalating safety concerns in a timely manner, and by doing so acting as effective advocates for those who use our services.

Experience

Essential criteria
  • Previous experience of working in the UK or a similar health economy
  • Experience (pre / post registration) in a variety of core areas to include the following: • Completion of core student placements: Respiratory/ Neuro/care of elderly/ MSK
  • Experience of working with students or assistant staff
  • Knowledge of general conditions seen in Physiotherapy including respiratory, outpatients, orthopaedics, neurological and care of the elderly multiple pathology
Desirable criteria
  • Experience (pre / post registration) in a variety of core areas to include the following: • Community Therapy
  • Experience (pre / post registration) in a variety of core areas to include the following: •Working in health or social care
  • Experience of involvement in departmental duties

Knowledge

Essential criteria
  • Understanding of the principles of Multi-Disciplinary Teamwork
  • Demonstrates an understanding of holistic rehabilitation of clients with complex needs and disability
  • Understanding of the political and economic situation in relation to health and social care
  • Understanding the Mental Capacity Act and applying as appropriate
  • Understanding of delirium and its effects
  • Understanding frailty

Education/ Qualifications

Essential criteria
  • Degree or equivalent Diploma in Physiotherapy
  • Current Health Professions Council Registration
  • Fluent in written and spoken English
Desirable criteria
  • Membership of The Chartered Society of Physiotherapy or any special interest group
