Job overview

To provide high-quality, proactive and person-centered clinical assessment, intervention and care coordination for care home residents with moderate and severe frailty and/or complex health needs. The post holder will act as the clinical delivery arm of the multidisciplinary Care Home Support Team, translating MDT decisions and anticipatory care plans into timely hands-on care within the care home.

A key priority is to improve the completion, quality and review of Urgent Care Plans (UCPs), ensuring that residents have a clear, personalised plan for recognising deterioration, treatment and escalation, including how to access appropriate community services and when hospital admission is or is not appropriate.
