Trosolwg o'r swydd

A GP with a specialist interest in diabetes managing the care of patients with diabetes within a community care setting, working alongside a multidisciplinary team to provide comprehensive care and support. 

The post holder will contribute to the Kingston Tier 3 Diabetes team by: -

·         Delivering evidence-based practice for patients with Type 2 Diabetes in the community with Multidisciplinary Team clinics across the borough of Kingston.

·         Working collaboratively with the Kingston Tier 3 Multidisciplinary Team and others to meet the needs of patients and carers.

·         Supporting and providing mentorship to other team members and Primary Care colleagues.

·         Contributing towards the service achieving its Key Performance Indicators.

 

This post is 3 sessions per month and will focus on a predominantly clinical role within the diabetes community team. The successful applicant will work alongside the diabetes specialist nurses who is delivering the service delivery role.

 

Prif ddyletswyddau'r swydd

·         To assess and treat new and follow up patients with Type 2 Diabetes referred from primary care

·         . The focus will be on educating the patient and their family and devising an appropriate management plan to be delivered by the specialist team in partnership with the patient and their primary care team

·         Management plan to include glycaemic control, cardiovascular risk and with consideration to each individual patient’s goals

·         To contribute to multidisciplinary reviews, care planning, clinical reports and referral to other appropriate agencies/services.

·         To provide health education advice for clients following the medical assessment / review.

·         To communicate and work with members of the wider diabetes specialist team to further develop the patients care plan

·         To follow NICE and SWL ICB and local pathways in the recommendations for treatment and technology for patients

·         To communicate effectively with service users, carers, families and other staff and professionals utilising forms of communication appropriate to the recipients.

·         To develop effective working relationships with a range of professionals.

·         To communicate appropriately with others within and outside the NHS in accordance with the Trust’s relevant policies.

·         To attend and contribute to relevant professional and service meetings and other multidisciplinary and multi-agency meetings.

  • To work autonomously within professional guidelines and the overall framework of the service’s and Trust’s policies and procedures.
  • To be accountable for own professional actions in terms of working with patients, relatives and other professionals.
  • To exercise autonomous professional responsibility for the medical assessment, monitoring and discharge of clients referred to the service.
  • To exercise expert professional judgement on the sharing of confidential information with other professional colleagues and agencies under the Sharing of Information Protocol, Information Governance, Safeguarding of Vulnerable Adults, Child Protection policies etc. in all aspects of clinical and managerial activity.
  • To receive regular clinical supervision from a more senior team member and to use this effectively to inform and further develop clinical practice and specialist knowledge.
  • To exercise responsibility for professional self-governance, in accordance with relevant professional codes of practice and Trust policies and procedures.
  • To maintain relevant professional registration and accreditation.
  • To contribute to multidisciplinary assessments, care planning, clinical reports and referral to other appropriate agencies / services if necessary.
  • To be sensitive to confidentiality and ethical issues as outlined in policies and procedures distributed by the Trust and professional body.

Experience

  • Practical experience as a Primary Care Practitioner.
  • Some previous experience of medical assessment and management of patients with Diabetes, and a keen desire to develop more specialist expertise in this field.

  • Working within specialist diabetes services in the community or secondary care

Skills / Abilities

  • Ability to work as a member of a team
  • The ability to manage a demanding workload and work well under pressure
  • High level of interpersonal skills
