Job overview

This is a fantastic opportunity to work at Croydon University Hospital as a ACE GP Extensivist one fixed day per week (Tuesday, 09:00–17:00).

This post is for a General Practitioner with an interest in frailty and interface care to work within the Acute Care of the Elderly (ACE) clinic.

The ACE clinic provides rapid-access comprehensive geriatric assessment (CGA) for older adults with frailty, aiming to deliver the right care in the right place,

avoid unnecessary hospital admission, and support safe community-based management.

Main duties of the job

The GP extensivist will apply advanced generalist skills, and further skills in frailty and CGA, to manage diagnostic uncertainty, complexity and risk at the frailty

interface. The role focuses on holistic medical assessment, optimisation of multimorbidity, medication rationalisation, and coordination of onward care. The service

supports admission avoidance, early supported discharge and continuity across organisational boundaries. 

Referrals are accepted from GPs, Emergency Department, inpatient wards, Rapid Response and community teams. The clinic is co-located with the ACE inpatient frailty unit and operates within an integrated care model.

The GP Extensivist works within a consultant-led ACE service. Consultant geriatricians are available for advice, escalation and shared decision-making at all times.

Clinical responsibility is shared within the ACE team, with clear escalation pathways for complex or high-risk decisions.

Qualifications

Essential criteria
  • GMC registration with licence to practise; CCT in General Practice
Desirable criteria
  • Postgraduate qualification in frailty or geriatric medicine; MRCP

Experience

  • Experience managing frailty, multimorbidity or complex older adults

  • Experience in interface, acute or community geriatrics
