An exciting opportunity has arisen for a Band 4 Patient Pathway Coordinator to join the Gastroenterology and Hepatology team within POD 3.

As our services continue to evolve, with new and innovative pathways being introduced across Hepatology and Liver services, we are seeking a motivated and organised individual to play a key role in supporting patients throughout their healthcare journey.

The successful post holder will work predominantly with the Hepatology and Liver team, while also providing administrative support across the wider Gastroenterology service as required.

Working as part of a multidisciplinary Point of Delivery (POD) team, the Patient Pathway Coordinator will provide proactive and comprehensive administrative support to ensure patients progress smoothly through their pathway from referral to discharge. The post holder will be responsible for coordinating appointments, managing waiting lists, monitoring pathway milestones, and ensuring that patient care is delivered in a timely and efficient manner.

As a key point of contact for patients, families, clinicians and colleagues, the post holder will provide excellent customer service and help ensure a positive patient experience by supporting all administrative aspects of care.

The post holder will act as the main point of contact for all administrative matters relating to patients’ pathways of care. They will work as part of a multidisciplinary team, providing expertise in the proactive management of patient pathways from referral through to discharge, ensuring processes run smoothly and efficiently.

The post holder will also be responsible for supervising and allocating work across the POD (Point of Delivery) team. They will promote high levels of patient and clinician satisfaction by being an accessible, customer-focused, and knowledgeable point of contact.

In line with Trust standards, the post holder will follow Standard Operating Procedures (SOPs) to ensure all aspects of the role are carried out accurately and within agreed timescales.

Qualifications

  • NVQ Level 4 in Business Administration/Customer Care or equivalent experience
  • Educated to GCSE/ O level or equivalent including Grade C or higher in Maths and English
  • Understanding of 18 Week RTT pathways and targets
  • Understanding of Cancer waiting time rules/target and performance indicators
  • Excellent Written and verbal communication skills
  • Exceptional Customer Service skills

  • Evidence of on-going continuous personal development (CPD) and training
  • NHS / Health Care Experience
  • Proficient in Microsoft word packages

Experience

  • Knowledge of healthcare administrative systems and processes (Experience of scheduling outpatient appointments and/or admissions)
  • Experience of using full range of IT systems and patient data systems (including advanced understanding of MS Office packages, Cerner/ERS trained)
  • Knowledge of NHS patient pathways - RTT & Cancer
  • Experience of prioritising own workload
