Trosolwg o'r swydd

Community Administrative Officer 

The post holder will provide a day to day co-ordination and administrative support in the single point of referral team and New Community Booking Team. She/he will act as a focal point for communication between the nursing staff, patients, hospitals, GP’s specialist nurses, allied health professionals, social services and other multidisciplinary teams referring into the service. Book community appointments, home visit slots and follow up activity in line with Trust procedures and local service requirements.

Support referral processes, including handling electronic and paper referrals and updating tracking systems.  Maintain accurate appointment information, clinic availability and patient details on Electronic Patient Record (EPR) systems.       Liaise with community clinicians, therapy teams, and other admin colleagues to support smooth patient flow.  Process appointment changes including cancellations, rescheduling and confirmation of new times. Support waiting list processes, highlighting any concerns or delays to senior administrative staff
