Job overview

Physical health advice and support to PWLD residing in residential care as part of the Enhanced Health in Care Homes Framework - NHS Long term Plan.

Main duties of the job

The postholder will support residential care home staff to proactively  manage and meet the health needs of individuals living in residential care.

The postholder will work collaboratively to ensure people living in care homes have access to enhanced primary and acute care and  referrals to specialist Learning Disability Services.

The postholder will also advice and support to ensure residents maintain their independence by reducing, delaying or preventing the need for additional health and social care services.

Essential criteria
  • RNLD
  • Knowledge of current research and legislation in the field of learning disability Nursing and ability to translate evidence into practice
  • 2 years post-registration experience of learning disability nursing
  • Experience of working with people with learning disabilities with complex physical health needs.
Desirable criteria
  • Evidence of post qualification training experience up to MA level in a relevant field
  • Evidence of further associated qualifications/CPD in Learning Disability Practice.
  • Experience of working with people with learning disabilities who engage in behaviours that challenge
  • Experience of assessing people with suspected dementia and autism
  • Experience of nursing in the community/CLDT
  • Ability to communicate effectively across organisations and disciplines at different levels

  • Sound knowledge of the principles of clinical governance, national policy and good practice guidance relating to people with learning disabilities. In particular, evidence based practice, clinical audit and the current access to healthcare agenda.
Desirable criteria
  • Experience of working with people with a Learning Disability in an acute/hospital setting.
