Job overview

Are you a Band 5 Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 5 nurse to support the changing face of community health care. As part of the Urgent Community Response and Virtual Ward initiative, your role will be essential in providing efficient, integrated, urgent care to our patients with care and respect.

 

Main duties of the job

You will be working as part of the Primary Care at Home team predominantly on Unplanned Care to triage and assess referrals into the service; You will be accountable for initial history taking, clinical assessment, triage and onward referral. There is an expectation that with demand in planned care you would be flexible to cross cover.

Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.

You will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community

Detailed job description and main responsibilities

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

 

You must have a driving license and have access to a vehicle in order to carry out your duties.  You will be required to work a shift pattern covering 7 days a week from 08:00-20:00.

  • Catheterisation 
  • bladder scanning,
  • End of life care
  • Medication administration
  • Syringe drivers
  • IV, cannulation,
  • Assessment post fall,
  • Minor injury assessment
  • Mound care
  • Observations
  • Venepuncture
  • Use of lifting equipment ie. raizer chair
  • Use of systmone

Person specification

Education

Essential criteria
  • Registered General Nurse
  • Demonstrate evidence-based practice and up-to-date clinical skills
Desirable criteria
  • Community Experience
  • Evidence of post-basic professional development

Experience

Essential criteria
  • Significant/recent experience working at a high level (Band 5 or above)
  • Ability to remain calm whilst working under pressure

Other

  • Driving licence
  • Access to a vehicle for work
