MAIN DUTIES AND RESPONSIBILITIES
1. CLINICAL RESPONSIBILITIES
Be trained and conversant in care flow EPR to ensure that all patient
admissions, discharges, and attendances are recorded accurately on Care flow
or the relevant electronic system in real time.
To assist with patient discharge arrangements by, liaising with other healthcare
providers, arranging outpatient appointments, and ensuring that relevant
paperwork is available, complete and accompanies the patient.
Communicate with a range of healthcare professionals using persuasive skills
when needed to facilitate effective discharge from hospital.
To work as part of a team alongside other WLO within the department and to
be part of a wider multidisciplinary team delivering a high-quality service to
other staff and patients. This includes working with identified external contacts
and stakeholders.

Main duties of the job
The post holder will provide high quality front line administrative support to the discharge planning team. The role involves collaborating closely with nurses and social worker, attend meetings and update patient flow / records.
Review and update daily discharges, update delay reasons in patient flow, confirm discharges with ward. Answer phones and manage the transfer of care mail box, allocate queries to relevant persons. Attend Teams meetings and update patient flow, filter and circulate Non Criteria to reside list to system partner. Submit NHSE weekly discharge Sitrep report weekly ( Monday). Provide additional administrative support to the transfer of care team

- Develop and provide a professional level of competency with relation to the Administrator role following an induction period.
- · Support the Discharge planning team and wider hospital services in a professional, and courteous manner, resolving issues where possible or escalating to an appropriate team member when required.
- · Communicate effectively and use appropriate interpersonal skills with people both internally and externally, always maintaining the professional reputation of the Trust and department.
- · Portrays a professional image, always maintaining confidentiality.
- · To continuously contribute to the progression and development of the discharge planning department through the provision of a high-quality service.
- The post holder will be expected to complete all mandatory training provided.
