Job overview

This is an exciting role providing clinical input to the management of patients on the Virtual Ward (VW) and an opportunity to shape this innovative service as it continues to develop.

The North Middlesex VW has been operating since 2022 and currently has 32 Main Ward and 8 Remote Monitoring beds. VW is supported by a team of ACP Band 7 Nurses, therapists and a Doctor. There is anticipated expansion into a number of new pathways during 2024/25 and a plan to increase both capacity and occupancy.

Support will be provided by the Divisional Medical Director for North Mid in the Community.

Main duties of job

Clinical responsibilities for this post will include a daily 2 hour long MDT; referral management, post admission review, clinical advice and support to the team of Nurses and therapists. This role will be responsible for maintain high clinical standards in the management of all virtual ward in-patients and liaising with specialty teams to ensure timely specialist review.

Service development activities will include: discussions with specialty leads, development and review of proposed pathways and participation in quality improvement.

The role will also have a strategic component: fostering relationships within the North Middlesex acute hospital team, community teams, sector partners and primary care to encourage uptake and participation in development of the virtual ward agenda.

Detailed job description and main responsibilities

This is a new  post based at North Middlesex University Hospital with some requirement for home visits to patients within the local area.The Virtual Ward is part of the Community Division with close co-working with the Medicine and Urgent Care Division.

DCC will consist of:

·       Daily Virtual Ward MDT

·        Post admission review

·         Review of referrals

·         Clinical advice and support

·         With scope for involvement in other specialties in the Trust – to be discussed

Person specification

Education & professional Qualifications

Essential criteria
  • MBBS or equivalent GMC Registration
  • Evidence of ongoing CPD and appraisals in line with GMC.
Desirable criteria
  • MRCP
  • Knowledge of national virtual ward frameworks and models

Professional Experience

Essential criteria
  • 4 years NHS experience or equivalent post GMC registration.
Desirable criteria
  • Evidence of previous audit and quality improvement work

Audit

  • Evidence of previous audit and quality improvement work
  • Able to identify improvement opportunities in a fast changing and evolving new service

Reseach

  • Understanding of research methodology and clinical paper interpretation
