Job overview
We are looking for an experienced band 7 Specialist Nurse to join our Diabetes and Endocrine team, to provide excellent inpatient and outpatient diabetes care cross site. The post holder will provide clinical expertise both in inpatient and outpatient care, and will join a dynamic team based at WMUH and Chelsea & Westminster Hospital.
Main duties of the job
- Working as an integral member of the diabetes team across Chelsea & Westminster and West Middlesex University Hospital.
- Active participation in nurse led / multidisciplinary clinics which would include:- Complicated cases, initiating and titrating new therapies (OHAs and insulin). Insulin pumps (working closely with our pump co-ordinator) aa well as initiating pump therapy with support, Adolescent transition clinics (including pump clinic), Pre-conception care, Ante-natal care, CGMS clinics and routine follow up.
- Nurse led Virtual – email / telephone clinics, including the safe logging of information in line with Trust guidelines.
- Providing skilled assessment, planning, delivery and evaluation of specialist diabetes care within defined guidelines and protocols.
- Provide advanced diabetes specialist nursing care to patients who have diabetes throughout the Trust, as well as offering structured education to patients.
- Ordering, performing and interpreting relevant tests and investigations.
- Providing expert diabetes advice and guidance to nurses & other disciplines, and teaching on nurse induction and mandatory update programmes.
- Providing specialist training and education to own and other professions.
- Setting and monitoring standards of diabetes care and treatment.
- Participating in audit and research activity.
- Disseminating good practice.
- Providing skilled professional leadership.

Detailed job description and main responsibilities
We are looking for an experienced band 7 Diabetes Specialist Nurse to join our Diabetes and Endocrine team, to provide excellent inpatient and outpatient diabetes care cross site. The post holder will provide clinical expertise both in inpatient and outpatient care, and will join a dynamic team based at WMUH and Chelsea & Westminster Hospital.
We are looking to expand our multidisciplinary team and are keen to receive applications from all Allied Healthcare Professionals as well as Clinical Nurse Specialists.
As a growing service meeting the needs of an expanding local population, the service provides a number of routine and specialist services, including; providing multidisciplinary clinic support, nurse-led clinics, ante-natal and pre-conception clinic, insulin pump clinic, transition clinic, virtual clinic, CGMS, emergency reviews and managing routine and urgent ward referrals. The team also carry out trust teaching sessions on a monthly basis for ward colleagues. We are also a DAFNE centre.
Applicants should possess an in-depth clinical knowledge of diabetes, which should be supported by recent experience/qualifications and the ability to initiate and titrate insulin and oral diabetes medications competently. This is an excellent opportunity for the right person to develop their career in an interesting and varied role. We aim to have all our diabetes specialist nurses to cover all aspects of diabetes care.

Previous Experience

- Experience of working in inpatients and outpatients relevant to the specialism
- Experience of working autonomously/ independently and as part of MDT
Desirable criteria
- Clinical supervision experience
- Insulin pump experience and management – certificate in insulin pump management
- Experience of antenatal diabetes care
Skills, Knowledge and Abilities
Essential criteria
- Excellent written & verbal communication and interpersonal skills
- Computer literate: able to use word, email, excel and manage databases
Desirable criteria
- Ability to recognize the contribution of members of the MDT
- Experience of audit, service improvement or research
